Compensation- Base Salary: $75,000-$90,000+ (based on experience)

What You'll Do- Develop and maintain relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, case managers, and other referral sources.
- Increase home health admissions and achieve monthly sales goals.
- Execute strategic territory management plans.
- Educate referral partners on Concierge Home Care's services.
- Coordinate smooth patient transitions from hospital or facility to home.
- Serve as a trusted healthcare resource within the community.
- Collaborate closely with clinical leadership to ensure outstanding patient outcomes.
- Maintain CRM documentation and referral activity.
- Represent Concierge Home Care at networking events and community outreach opportunities.
QualificationsRequired- Healthcare sales experience or strong clinical experience with existing healthcare relationships
- Excellent communication and relationship-building skills
- Self-motivated with the ability to work independently
- Valid driver's license
- Reliable transportation
- Auto insurance
Preferred- Existing referral relationships within Bay or Gulf County
