Qualifications

• MSN or higher-level Adult Nurse Practitioner required.
• Licensure for advanced nursing practice in KY with DEA and controlled substance permits required.
• Minimum of one year of full-time advanced nursing experience required.
• Previous experience in assisted living or home-based primary care preferred.
• Experience working with the geriatric population preferred.
• Proficient with electronic medical records and technology-based tools.
• Must possess a valid driver's license.

Responsibilities

• Serve as primary care clinician for patients in home and facility settings.
• Plan, direct, and evaluate patient care for various medical conditions.
• Conduct assessments, make diagnoses, and treat acute and chronic illnesses.
• Perform diagnostic testing, develop treatment plans, and prescribe medications.
• Coordinate referrals and collaborate with interdisciplinary care teams.
• Ensure compliance with established protocols and scope of practice.
• Fulfills on-call duty policies, which may involve after-hours and weekend duties.
