Schedule: Monday-Friday 8:00am to 5:00pm with occasional weekend and weekday afterhours on-callPosition Summary: The Home-Based Primary Care Nurse Practitioner serves as the primary care clinician for a patient panel residing within homes, assisted living facilities and personal care facilities. The Nurse Practitioner plans, directs, and evaluates patient care including the assessment, diagnosis, and treatment of chronic conditions, acute illnesses, and episodic medical needs within the scope of advanced nursing practice. This role performs selected diagnostic testing, develops treatment plans, prescribes medications, coordinates referrals, and performs procedures within the scope of licensure, clinical expertise, established protocols, and available equipment. The Nurse Practitioner collaborates with the supervising physicians, facility staff, and interdisciplinary care teams to provide high- quality, patient-centered care across home-based settings.
Position Qualifications: - MSN or higher-level Adult Nurse Practitioner required.
- Licensure (or eligibility for licensure) for advanced nursing practice in KY, including DEA and controlled substance permits, required.
- Minimum of one year of full-time advanced nursing experience required. Previous experience in assisted living or home-based primary care and experience working with geriatric population preferred.
- Fulfill requirements of on-call duty policies.
- Proficiency using technology-based tools, including electronic medical records and other software programs.
- Must be a licensed driver.
