The Role

Shares specialized knowledge and leadership through consultation and collaboration to improve resident outcomes and health care delivery for residents with medically complex needs.

Duties/Responsibilities

  • Practices autonomously, provides comprehensive health assessments, diagnoses health/illness conditions, and treats/manages acute and chronic illnesses.
  • Collaborates with the Medical Director and Manager of Resident Care in the provision and advancement of nursing services.
  • Collaborates with all attending physicians.
  • Plans strategically and operationally to address the changing and increasingly complex care needs of the residents.
  • Provides ongoing formal teaching and coaching in the management of clinical care to interdisciplinary team members, caregivers, volunteers, visitors, and residents, serving as a resource, educator, and role model regarding protocols and related issues.
  • Assists and provides coverage for day-to-day human resource functions, including hiring, disciplining, training, and coaching staff to ensure optimal performance.
  • Directs and supervises nursing staff for compliance. Works to develop a culture of excellence in clinical practice using a person-centred and emotion-focused care philosophy. Effectively responds to emergent and confidential human resources issues as required.
  • Performs a key role in various clinical programs, including skin and wound care, infection control, pain management, palliative care, and various quality improvement initiatives. Ensures the use of best practices and industry standards.
  • Provides clinical contributions in the development of resident care plans, care conferences, and implementing and evaluating clinical decisions.
  • Assists in coordinating nursing student placements, including preceptorship roles, evaluation, and linking college/university programs with divisional opportunities. Assesses student learning objectives and considers established program needs to identify opportunities.
  • Assists in the review of applications from the Home and Community Support Services. Conducts site visits for potential bed offers.
  • Participates in program staff meetings and orientation.
  • Performs related duties as required.


Knowledge, Skills, and Abilities

  • Knowledge and skills are acquired through a Baccalaureate in Nursing and proof of current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
  • Experience in gerontological nursing and/or as a primary health care nurse practitioner.
  • Successful completion of the Ontario Primary HealthCare Nurse Practitioner Certificate Program, with ability to participate in continuing education activities to maintain licensing as a Nurse Practitioner.
  • Current Membership in the Nurse Practitioners Association of Ontario and the Registered Nurses Association of Ontario (RNAO).
  • Knowledge of the Standards of the College of Nurses of Ontario, LTC Standards, Pharmacy Standards, and the Regulated Health Professionals Act.
  • Knowledge of infection control and risk management.
  • Current Canadian Gerontological Nursing certification or current plan to obtain certification.
  • Leadership skills to supervise, train, develop, motivate, and support staff.
  • Human relations and communication skills to resolve conflict; respond to complaints and inquiries from residents, families, and designated decision-makers; and participate as an effective team member.
  • Ability to travel within Waterloo Region.
  • Computer skills with ability to use software such as Microsoft Office for data entry, case management, and document preparation. Skill in Internet-based searching and networking.
  • Must have a tuberculosis (TB) test and all required immunizations.
  • Must provide an acceptable vulnerable sector police check.
  • Ability to participate in a weekend supervisory rotation and official standby/on-call rotation to respond to legislated or time sensitive demands beyond regularly scheduled work hours.
  • Ability to implement and monitor compliance with the vision and philosophy for person-centered and emotion-focused care.
  • Ability to support and demonstrate the values of Seniors' Services and the Region.
