Description

The Gardens at St. Elizabeth has an exciting new opportunity for a Health and Wellness Manager for our Assisted Living and Memory Care community.

The Wellness Coordinator is responsible for the overall leadership and daily operations of the Assisted Living community. This position provides oversight of resident care, resident experience, regulatory compliance and employee oversight. Responsibilities include the development, implementation, and evaluation of policies, procedures, and standards of practice in collaboration with the CHI Living Communities.

Working collaboratively with the Executive Director, interdisciplinary team, residents, families, physicians, and community partners, the Wellness Coordinator promotes resident-centered care that enhances the health, safety, dignity, and quality of life of each resident. This position is responsible for ensuring compliance with Ohio Administrative Code, CHI Living Communities policies and procedures, and all applicable federal, state, and local regulations while fostering a culture of excellence, accountability, education, and continuous quality improvement.

Job Responsibilities

  5. Satisfies all educational in-service requirements mandated by CHI Living Communities, the
  6. department, external accrediting, and regulatory agencies.
  7. Evaluates resident/family needs in relation to resources and refers to appropriate agency or resource


Job Requirements
  • Current Nurse license in the state of Colorado, RN
  • Currently certified in CPR and First Aid
  • Long-term care experience preferred
  • Point Click Care experience preferred
