Position Summary

St. Luke's Health System in Twin Falls, Idaho is seeking a Full-time Outpatient Pharmacist to join our outpatient pharmacy. This position plays a key role in delivering high-quality pharmaceutical care across multiple outpatient service areas, including medication lockers, traditional outpatient pharmacy services.

The ideal candidate will be comfortable with the clinical aspects of pharmacy practice, thrive in a collaborative healthcare environment, and be motivated to support innovative outpatient pharmacy services.

Key Responsibilities

The Outpatient Pharmacist is responsible for the delivery of safe, effective, and patient-centered pharmaceutical care, including but not limited to:
  • Review and evaluate the appropriateness of prescribed medication therapies
  • Interpret prescriptions and accurately compound and dispense medications
  • Provide medication information, counseling, and consultation to patients, nurses, and medical staff
  • Perform cognitive clinical pharmacy services and document interventions according to medical center standards
  • Administer medications and vaccinations as appropriate
  • Supervise, direct, and authorize the work of pharmacy technicians, interns, externs, and support staff
  • Maintain accurate records related to medication therapy, prescriptions, patient charges, inventory, and medical records
  • Support Meds-to-Beds and Transitions of Care services to improve patient outcomes and continuity of care
  • Assist with training and education of technicians, interns, externs, residents, and other pharmacy staff
  • Serve as a role model and mentor to students, technicians, residents, and new practitioners

Qualifications

Education
  • Degree from an accredited School or College of Pharmacy (PharmD or equivalent)

Experience
  • 0 years of experience required
  • Experience with EPIC electronic medical record system is preferred

Licenses & Certifications
  • Valid Pharmacist License in the state of practice (Idaho or eligible for licensure)
  • Basic Life Support (BLS) certification required, through one of the following:
    • American Safety and Health Institute (ASHI)
    • American Heart Association (AHA)
    • American Red Cross
