Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• One to two years nursing experience preferred.
• Community health or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the State(s) of planned practice.
• Must be a licensed driver with insured automobile in good condition.
• Possesses current CPR certification.
• Knowledge of geriatric population needs.
• Capable of physical demands of nursing, including lifting and prolonged standing.

Responsibilities

• Provides direct patient care under RN supervision following the plan of care.
• Documents patient services accurately and timely to reflect care plans.
• Educates patients and caregivers about diagnoses and care plans.
• Prepares and uses medical equipment and supplies efficiently.
• Manages patient pain and symptoms, evaluating treatment responses.
• Engages in ongoing professional development and quality improvement activities.
• Reports significant patient condition changes to the RN Case Manager.
