Role and Facility Information

Now is the time to augment your practice with a long term care (LTC) panel to provide the challenging, diverse, and impactful work of caring for the frail elderly. LTC panels are permanent, part-time placements for family physicians.

Why long term care?
- Specialized mental health population; expand on your mental health skillset and apply your expertise to a LTC setting
- You will become part of a supportive physician community
- Each care home works closely with an interdisciplinary care team to provide holistic resident-centered care
- You have the opportunity to join the Long Term Care Initiative (LTCI) which includes: paid mentorship, participation in a shared after-hours care system, and paid participation in LTC-specific events and projects. Joining the LTCI represents a formal commitment to the care home and meeting the five best practice expectations which include: regular proactive visits; on-site medication reviews; attendance at care conferences; involvement in 24/7 on call; and completed documentation in charts
- You can develop and grow your skills in providing care for the frail elderly and transfer these skills to your work in community and hospital settings
- Allows for flexibility in your schedule - supplement other family practice work with a LTC panel to add variety to your workday. You can work with the care home leadership to set the size of your panel and your availability for providing care

Remuneration
LTC panels are remunerated through the LFP model or fee-for-service. Physicians who join the LTCI also receive payments for:
- Participation in the after-hours care program: $1,000 for weekday shifts and $1,400 for weekend shifts
- Sessional payments for participation in mentorship activities, events, and quality improvement projects

Care home information
Britannia Lodge is a 65-bed long term care home currently being supported by two Long Term Care Initiative (LTCI) practitioners. An additional practitioner is required to assist with the resident caseload.
