Family Nurse Practitioner - Mobile Clinic Program ChampionDepartment: MHCS Department
As a Family Nurse Practitioner, you will:
- Primary Care Services: Conduct physical exams, diagnose and treat acute and chronic illnesses, and provide routine primary care to improve patient health outcomes.
- Patient Assessments: Take medical histories, perform physical examinations, and order and interpret diagnostic tests.
- Chronic Disease Management: Develop and manage treatment plans for chronic conditions such as diabetes, hypertension, and asthma.
- Documentation: Maintain accurate and timely patient records using electronic health record (EHR) systems.
- Administrative Duties: Including but not limited to assisting with opening/closing the clinic, inventory, patient follow-up, verifying insurance, collecting payment, scheduling appointments, and maintaining a clean environment.
- Preventive Care: Administer vaccinations, conduct health screenings, and educate patients on disease prevention and healthy lifestyles.
- Collaboration: Work with other healthcare providers, nurses, and medical assistants to deliver comprehensive care.
- Community Outreach: Participate in outreach activities to raise awareness about the mobile clinic's services and engage with the community.
- Actively participating in leadership-level planning for the best growth opportunities.

- Education:
- Master's or DNP Family Nurse Practitioner (FNP) with completion of an accredited FNP program, with current Family Nurse Practitioner (ANCC or AANPCB) Board Certification or equivalent certification.
- Currently licensed or eligible for California APRN licensure as a Family Nurse Practitioner
- Experience:
- Clinical experience in family and primary care servicespreferred
- Experience with EHR preferred
- Performance: Able and willing to provide world-class service to all stakeholders

Work Environment - 100% Onsite
- Travel to various community locations, often in underserved or rural areas
- Flexible working hours, including some evenings or weekends, depending on the clinic's schedule
- Regular work on and around the mobile unit
- Community-facing role requiring strong interpersonal communication and professionalism
- Fast-paced environment with outdoor work and varied weather conditions
- When mobile clinic events are not scheduled, this position will operate as a Family Nurse Practitioner within the brick-and-mortar clinic as needed
Salary & Benefits - 80 PTO hours available after the first 60 days of service
