Shift Type: Full-Time, Monday-Friday, during normal business hours, flexible, and on-call rotations.
Compensation: Guaranteed base Salary, Per point incentive pay in addition to the base salary, Earning Potential of $3,456 - $3,625+ bi-weekly, Payroll processed bi-weekly (26 pay periods), Mileage reimbursed at $0.67/mile

Summary:
We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient's care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.
Responsibilities:
- Assumes responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.
- Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
- Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
- Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
- Participate in on-call rotation as reasonably assigned.
Qualifications:
- Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
- Active State Registered Nurse licensure is required.
- Proof of current CPR certification and valid Driver's license.
