Early Intervention Nurse
Nursing actions are directed toward assessment, treatment, case management, collaboration with professionals, and providing community resources to families in Summit and Wasatch County. Nursing will follow the policies of the Baby Watch Early Intervention Program (BWEIP) with the state health department. Open until filled.

SALARY RANGE: $70,791.48 - $99,222.98/annuallyMINIMUM QUALIFICATIONS:- Minimum of an Associate Degree in Nursing, Bachelor Degree in Nursing preferred.
- Two (2) years experience in Clinical Nursing or an equivalent combination of education and experience.

- Must be CPR/AED certified withing three (3) months from date of hire.
- Must possess a valid driver's license issued by your state of residence.
- Must earn certification as an early intervention specialist credential within one (1) year from date of hire.
- May be required to attend continuing education courses or seminars as needed for pediatric nursing.

ESSENTIAL FUNCTIONS - Performs comprehensive nursing evaluation with children, ages 0-3 using the Baby Watch Early Intervention Nursing Health, Hearing and Vision (HHS) Assessment in all areas of development that are used to determine eligibility for services. Reviews medical documentation from physicians, neonatal intensive care units, and outside agencies at annual and periodic reviews.
- Assists to qualify children for eligibility by medical diagnoses, test scores, or clinical opinion. Serves as a service coordinator for families and children. Reviews with family's fee determination, procedural safeguards, parental rights and responsibilities including confidentiality, parental consent and notice, and dispute resolution procedures following BWEIP policy.
- Documents medical information in Baby and Toddler Online Training System (BTOTS) a secure database used by Utah EI programs and providers to record service information. BTOTS is designed to track EI activities, follow compliance with state and federal regulations, and support compliance and monitoring with statewide child information. Nursing will give team input for functional measurable goals on the IFSP (Individualized Family Service Plan) and provide nursing documentation for transition for the school district.
- Provides service coordination, targeted case management for families by linking to community resources in Summit and Wasatch County. Attend meetings 6 times a year for the Medical Home Model and provide quarterly updates to pediatricians. Communicate with DCFS about referrals and discuss plan for services.
- Completes family based intervention to build family support with health education to increase engagement and effectiveness in activities among children. The nurse will help to improve the well-being of the caregiver and reduce stress.
- Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, sexual orientations, lifestyles and physical abilities.
- Participates in Health Department and community emergency response training and drills as required by the Emergency Manager in support of the public health emergency and disaster preparedness, Performs public health emergency response duties as assigned and consistent with job responsibilities.
- Performs related duties as directed by the Director of Early Intervention or the Director of the Health Department.
