Practice Setting: Assisted Living, Memory Care & Senior Living Communities

We are looking for a dual-certified Family Nurse Practitioner (FNP) / Adult-Gerontology Primary Care Nurse Practitioner-Psychiatric Mental Health Nurse Practitioner (AGPCNP/PMHNP) who is passionate about caring for older adults and wants to practice at the intersection of geriatric medicine and behavioral health.
The Opportunity

This is an opportunity for an experienced, autonomous NP to develop meaningful relationships with older adults while providing comprehensive care inside senior living communities rather than in a traditional clinic setting.

You will serve as a trusted medical provider for a panel of residents, working closely with families, caregivers, facility leadership, nursing staff, social workers, hospice and palliative care teams, and other members of the interdisciplinary team.

Your dual certification allows you to address the whole person - including both physical and behavioral health needs - while avoiding fragmented care.
Responsibilities

• Provide comprehensive primary care to residents in assisted living and memory care communities
• Perform new patient evaluations, annual wellness visits, chronic disease management, and acute visits
• Diagnose and manage common geriatric conditions and complex multimorbidity
• Evaluate and manage depression, anxiety, behavioral symptoms, sleep concerns, and other common psychiatric conditions in older adults
• Provide medication management with particular attention to polypharmacy, deprescribing, and medication safety
• Participate in goals-of-care conversations and advance care planning
• Support appropriate hospice and palliative care referrals when indicated
• Coordinate care with families, MPOAs, facility nursing teams, specialists, hospice, and other community providers
• Manage acute changes in condition with an emphasis on treating patients in place whenever clinically appropriate
• Participate in interdisciplinary care planning
• Provide education and support to residents, families, caregivers, and facility staff
• Complete timely, high-quality clinical documentation
• Help promote evidence-based geriatric care and antibiotic stewardship
• Build strong relationships with senior living communities and become a consistent clinical presence for residents and staff

Ideal Candidate

We are looking for a provider who:

• Holds active FNP and PMHNP/AGNP certification or equivalent dual certification appropriate for the role
• Has an active Utah APRN license and prescriptive authority
• Has experience caring for older adults, preferably in geriatrics, assisted living, memory care, primary care, psychiatry, palliative care, or hospice
• Is comfortable practicing independently while knowing when to collaborate or seek consultation
• Has strong clinical judgment and excellent communication skills
• Is comfortable managing medically complex older adults
• Understands the importance of medication reduction and avoiding potentially inappropriate medications in older adults
• Is comfortable discussing dementia, capacity, advance directives, goals of care, and end-of-life care
• Values continuity and long-term relationships with patients and families
• Is organized, adaptable, and comfortable working outside of a traditional clinic environment
• Has a genuine passion for improving the experience of aging
