Description

We9re seeking the right candidate for the position of Director of Nursing at Trinity Trace Community Care Center. In this vital role, you9ll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support.

What You Bring to the Table:

Essential Qualifications:
  • Develop and maintain comprehensive care plans tailored to the needs of clients.
  • Coordinate services for individuals requiring senior care, home care, or support for people with disabilities.
  • Provide behavior management strategies and support for clients dealing with dementia or other cognitive challenges.
  • Assist with medication administration and ensure compliance with prescribed treatments.
  • Conduct regular assessments of client progress and adjust care plans as necessary.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure continuity of care.
  • Facilitate communication between clients and their families regarding care options and progress.
  • Maintain accurate documentation of client interactions, care plans, and any changes in condition.


Requirements

The Ideal Candidate:
  • RN with at least 2+ years of Assisted Living experience.
  • Experience in senior care, home care, or working with individuals with disabilities is required.
  • Knowledge of behavior management techniques and dementia care practices is a plus.
  • Strong skills in patient care and case management are essential for success in this role.
  • Ability to lift clients safely as required during caregiving tasks.
  • Excellent communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
  • Compassionate nature with a strong commitment to enhancing the lives of those in your care.
  • Relevant certifications or training in caregiving or case management are advantageous.
  • Graduate of an accredited school of nursing currently registered with the State agency for nursing licensure and holds a valid nursing license to practice in the state of Louisiana.


Other Qualifications:
  • Previous DON experience preferred.
  • Has not been excluded from participating in Medicare and Medicaid programs.
  • Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
  • Participate in pre-employment and annual Tuberculosis screening.
