Director of Nursing at
Trinity Trace Community Care Center. In this vital role, you9ll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support.
What You Bring to the Table:Essential Qualifications:- Develop and maintain comprehensive care plans tailored to the needs of clients.
- Coordinate services for individuals requiring senior care, home care, or support for people with disabilities.
- Provide behavior management strategies and support for clients dealing with dementia or other cognitive challenges.
- Assist with medication administration and ensure compliance with prescribed treatments.
- Conduct regular assessments of client progress and adjust care plans as necessary.
- Collaborate with healthcare providers, family members, and other stakeholders to ensure continuity of care.
- Facilitate communication between clients and their families regarding care options and progress.
- Maintain accurate documentation of client interactions, care plans, and any changes in condition.
RequirementsThe Ideal Candidate:- RN with at least 2+ years of Assisted Living experience.
- Experience in senior care, home care, or working with individuals with disabilities is required.
- Knowledge of behavior management techniques and dementia care practices is a plus.
- Strong skills in patient care and case management are essential for success in this role.
- Ability to lift clients safely as required during caregiving tasks.
- Excellent communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
- Compassionate nature with a strong commitment to enhancing the lives of those in your care.
- Relevant certifications or training in caregiving or case management are advantageous.
- Graduate of an accredited school of nursing currently registered with the State agency for nursing licensure and holds a valid nursing license to practice in the state of Louisiana.
Other Qualifications:- Previous DON experience preferred.
- Has not been excluded from participating in Medicare and Medicaid programs.
- Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
- Participate in pre-employment and annual Tuberculosis screening.
