The JobArrowhead Regional Medical Center (ARMC) is recruiting for a
Discharge Registered Nurse* to ensure safe and efficient transitions of care for medically cleared patients from hospital to home. The discharge nurse identifies patients who meet criteria for a safe discharge and facilitates the process and communicates with all departments as needed to maximize efficiency and workflow to accommodate safe patient discharges.
Duties include, but are not limited to, the following:- Prioritize safe patient discharges; Collaborate and communicate discharge care plans with the care teams to develop and implement a smooth patient discharge plan.
- Identify and minimize delays relating to patient discharges; Evaluate patient's readiness for discharge, ensuring they meet necessary criteria for a safe transition to home.
- Educate patients and their caregivers about post-discharge care, medication management, and follow-up appointments.
- Teach, delegate, and direct both professional and non-professional nursing staff and interpret the needs of each patient to all persons participating in patient care.
- Maintain accurate records in the Electronic Health Record (EHR) of discharge plans and patient interactions to ensure continuity of care.

CONDITIONS OF EMPLOYMENT

Availability: Twenty-four (24) hour responsibility, on-call and call-back may be required.

Minimum Requirements
Candidates must possess all of the following:
License: Must possess and maintain Registered Nurse licensure with the State of California Board of Registered Nursing.
Experience: One (1) year of full-time equivalent experience as a licensed Registered Nurse.
Certification: Must possess and maintain an Advanced Cardiovascular Life Support (ACLS) and Healthcare Provider level Basic Life Support (BLS) certification.
Desired Qualifications
The ideal candidate will possess experience as a Discharge Registered Nurse in an acute care hospital.
