Major Responsibilities:
  • Establishes the enterprise vision, strategy, governance framework, and future-state direction for the Virtual Social Care service line, ensuring alignment with enterprise social health priorities, virtual care strategies, and evolving care delivery models.
  • Selects, coaches, and develops Virtual Social Care leaders and team members, establishing performance expectations, fostering leadership development, succession planning, accountability, employee engagement, and a culture of continuous improvement.
  • Leads the design, implementation, governance, and continuous evolution of the enterprise Virtual Social Care operating model and Virtual Hub, including AI-enabled intake, patient triage, clinical routing, virtual capacity management, and access to social health interventions. Ensures the model is scalable to support future social care delivery strategies across virtual, hybrid, and community-based service models.
  • Establishes enterprise standards for Virtual Social Care workflows, patient routing criteria, escalation pathways, quality expectations, and virtual service delivery practices while delegating operational implementation and workflow maintenance to supporting operational teams.
  • Partners with Information Technology, Epic, Analytics, Artificial Intelligence, Virtual Care, and operational leaders to define technology strategy, prioritize capability development, and advance innovative solutions that enhance virtual social care, patient experience, workflow efficiency, and service line scalability.
  • Maintains accountability for Virtual Social Care service line performance through oversight of patient access, quality, productivity, capacity utilization, and service outcomes, leveraging data to drive strategic decision-making and organizational results.
  • Leads enterprise planning and innovation activities related to virtual social care delivery, AI-enabled service models, automation opportunities, emerging technology capabilities, and future-state expansion of the Virtual Social Care service line.
  • Builds and sustains strategic partnerships with executive leaders, market leadership, clinical stakeholders, operational departments, and technology teams to drive enterprise alignment, shared accountability, and successful implementation of Virtual Social Care initiatives.
  • Provides strategic oversight of Virtual Social Care staffing models, virtual capacity planning, workload balancing, resource allocation, and scalability strategies to ensure sustainable growth and efficient service delivery.
  • Champions innovation, digital transformation, and future-state planning by evaluating emerging technologies, AI-enabled capabilities, evolving care delivery models, and operational best practices to continuously advance the Virtual Social Care function.
  • Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that people actions, and the actions of supervised employees, comply with the policies, regulations, and laws applicable to the organization's business.
  • Provides executive oversight of the Virtual Social Care service line and Virtual Hub, ensuring alignment of enterprise standards, service expectations, operational performance, and patient access outcomes across markets.

Education Required:
  • Bachelor's degree in Healthcare Administration, Public Health, Social Work, Nursing, Business Administration, Health Informatics, Human Services, or a related field, or equivalent knowledge typically acquired through a combination of education, training, and progressive professional experience.


Experience Required:
  • Seven or more years of progressive experience in healthcare operations, population health, social health, care coordination, clinical operations, virtual care, or related healthcare leadership roles.
  • Minimum of three years of leadership experience with responsibility for enterprise program operations, organizational strategy, performance accountability, and cross-functional leadership.
  • Experience leading complex enterprise initiatives involving operating model design, technology implementation, workflow optimization, performance improvement, and executive stakeholder engagement.


Knowledge, Skills & Abilities Required:
  • Knowledge of enterprise social health strategy, Virtual Social Care operating models, healthcare operations, workflow governance, care coordination, population health, process improvement, performance management, healthcare technology platforms, electronic medical records, digital workflow solutions, and technology-enabled care delivery.
  • Strong strategic planning, operational leadership, executive communication, facilitation, stakeholder influence, collaboration, governance, organizational change management, and performance management skills.
  • Ability to establish enterprise vision, govern enterprise operating models, lead enterprise-wide initiatives across multiple stakeholder groups, influence executive and operational leaders, manage competing priorities, and drive measurable operational performance within a complex, matrixed healthcare environment.


Physical Requirements and Working Conditions:
  • Operates in a hybrid office and healthcare environment. Requires prolonged periods of sitting, computer use, participation in virtual meetings, and occasional travel between markets, facilities, and community partner locations. Must be able to communicate effectively with diverse stakeholder groups and perform responsibilities in a fast-paced, matrixed environment


Preferred Job Requirements:

Licensure, Registration, and/or Certification Required:
  • Board Certification in Laboratory Animal Medicine preferred.


Education Required:
  • Master's degree in Healthcare Administration, Public Health, Social Work, Business Administration, Health Informatics, Organizational Leadership, or a related field preferred.


Experience Required:
  • Experience leading enterprise virtual care, social health, community health, population health, patient navigation, or care coordination programs.
  • Experience managing enterprise operating model transformation, AI-enabled workflow implementation, and large-scale organizational change across a multi-market healthcare system.
  • Experience with Epic, artificial intelligence-enabled workflow tools, healthcare analytics, enterprise operating model design, governance structures, and technology-enabled care delivery.


Knowledge, Skills & Abilities Required:
  • Expertise in enterprise operating model governance, AI-supported workflow solutions, technology-enabled care delivery models, enterprise service line strategy, organizational transformation, scalability planning, and continuous improvement.


Advanced ability to influence executive leadership, align diverse stakeholder groups, lead large-scale organizational change, establish governance structures, and drive adoption of innovative care delivery models across a complex healthcare system.
