Summary:
Responsible for leading the operational execution and delivery of value-based care programs across Physician Enterprise. The director will ensure programs are implemented effectively, operational barriers are resolved and cross-functional teams are aligned to deliver results supporting enterprise goals and objectives. The director will leverage tools and technology to develop internal and external reports, complete program reporting and inform leadership as to the ongoing status of programs and performance. The director requires an innovative approach to prioritizing initiatives along with excellent project management, delegation, and communication abilities. The director collaborates with both clinical and administrative leaders to develop operational standards which improve quality outcomes by promoting best practices in the care and treatment of our patients while reducing costs.
The director will use industry benchmarks to recommend best-practice solutions for the development, execution, and measurement of initiatives. The director will participate in the planning, development and coordination of department-specific and cross-functional quality initiatives. Will oversee and develop staff as assigned and shall function to promote teamwork and accountability in a performance-based work environment.
Responsibilities:
EDUCATION- Bachelor's degree in healthcare administration, nursing, or clinical related field required
- Master's Degree is preferred
EXPERIENCE- Two (2) years prior experience in quality/performance improvement, process improvement, preferably in a post-acute care setting
- One (1) year experience in education and training
- Five (5) years relevant health care experience in a progressively responsible management or administrative position
- Community medical clinics and / or Ambulatory Care and formal education in process improvement experience desirable
KNOWLEDGE, SKILL AND ABILITY- Familiar with programs related to risk adjustment, quality measurement and financial performance to support value-based strategy
- Ability to analyze performance data to drive improvements in clinical outcomes and financial performance
- Prior experience of facilitating presentations to all levels of the organization is required.
- Superior organizational and interpersonal skills are required.
- The ability to work with individuals at all organizational levels, particularly physicians, peers, other departments, patients, and the community is required.
- Ability to manage projects and develop systems
- Skill in exercising initiative, judgment, problem solving and decision making
- Ability to effectively communicate in a professional manner with physicians, administrators, patients, and associates
- Ability to create an atmosphere which encourages motivation, innovation, and high performance to foster adoption of new care models
