Director of Wellness (LPN)

Assisted Living Community
Salary: $85,000-$90,000 per year

Qualifications
  • Current Licensed Practical Nurse (LPN) license in good standing with the state licensing board.
  • Previous leadership experience in Assisted Living, Memory Care, or Long-Term Care strongly preferred.
  • Thorough understanding of state regulations governing Assisted Living communities.
  • Strong clinical assessment, leadership, and organizational skills.
  • Experience supervising, coaching, and developing nursing staff.
  • Excellent communication and relationship-building skills with residents, families, physicians, and team members.
  • Ability to manage multiple priorities while maintaining exceptional resident care.


Primary Responsibilities
Clinical Leadership
  • Direct and oversee the day-to-day clinical operations of the Assisted Living community.
  • Promote a culture of compassionate, resident-centered care and clinical excellence.
  • Ensure all nursing services are delivered in accordance with physician orders, individualized service plans, and state regulations.
  • Monitor resident health status and coordinate appropriate interventions when changes occur.
Resident Wellness
  • Complete resident assessments and develop individualized wellness plans.
  • Coordinate care with physicians, pharmacies, therapy providers, hospice agencies, and other healthcare partners.
  • Oversee medication management and ensure safe medication administration practices.
  • Communicate effectively with residents and families regarding health concerns and care planning.
Team Leadership
  • Recruit, hire, train, mentor, and retain nursing staff.
  • Develop staff schedules to ensure appropriate clinical coverage.
  • Conduct performance evaluations and provide coaching and ongoing education.
  • Foster a positive, team-oriented work environment focused on quality care and resident satisfaction.
Regulatory Compliance
  • Ensure compliance with all applicable state regulations, company policies, and clinical standards.
  • Maintain accurate clinical documentation and medication records.
  • Prepare for and participate in state surveys and regulatory inspections.
  • Monitor infection prevention practices and quality assurance initiatives.
Collaboration
  • Partner with the Executive Director and department leaders to promote resident satisfaction and operational excellence.
  • Participate in resident move-in assessments and care conferences.
  • Serve as a trusted resource for residents, families, and team members.
