Full Job Description
Job Summary:

The Director of Transitional Care assumes responsibility and accountability for developing, coordinating, implementing and evaluating Transitional Care Initiatives. Specific duties include coordination of strategic and operational Transitional Care Initiatives, management and evaluation of Transitional Services operations, community outreach, collaboration with other health care agencies and facilities, customer and staff satisfaction, quality management certification and accreditation activities, program development and professional practice activities. The Director exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.

Minimum Qualifications:

Masters degree required with either Bachelor's or Master's degree in Nursing, NYS licensed/registered RN required OR Masters degree in Social Work & LMSW required. Minimum of 7 years progressively responsible nursing leadership experience which encompasses medical staff relationships, quality management, human resource development and management. Excellent written/oral communication, organizational, interpersonal, analytical, computer, and presentation skills required.

Preferred Qualifications:

B.S.N preferred. Experience in Transitional Care and certification as a Transitions coach preferred. Leadership experience in acute care environment or transitional services.
