Work Shifts8:00 A.M - 4:00 P.MDuties & ResponsibilitiesSUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:- Leads teams and system-wide efforts to improve key care processes and outcomes.
- Assists in the development of goals and objectives, policies, procedures, and protocols as they relate to performance improvement.
- Support departments in navigating Central Office-mandated Epic restrictions related to data extraction and visualization by providing data access, reporting, analytical, and visualization support.
- Develop and maintain data extracts, reports, dashboards, and visualizations to support departmental quality improvement, operational monitoring, and decision-making.
- Serve as a resource to departments that require assistance accessing, analyzing, and presenting Epic data within established system and governance requirements.
- Partner with the Office of Quality and Safety and departmental stakeholders to identify data needs, translate requirements into actionable reports and visualizations, and ensure timely delivery of accurate and meaningful information.
- Practice excellent employee relations with attention to employee needs and develop and mentor a high-performing team for all areas of responsibility.
- Serves as facilitator/ team member/ leader for assigned quality improvement teams and or committees.
- Works effectively across facility departments to facilitate improvements
- Abstracts, compiles, analyzes, and evaluates data to identify patterns or trends, using statistical processes and controls and various databases or software programs.
- Supports in resolving identified concerns or trends through facilitation, coordination, education, monitoring, and evaluation.
- Manages and participates in departmental projects, workflow processes, and review and/or creation of policies and procedures
- Facilitates process evaluation, standardization, implementation - integrating evidence-based care into clinical order sets, protocols, pathways, rules, and alerts in the electronic medical record (EPIC system experience preferred)
- Assures that improvement efforts incorporate all regulatory requirements into process specifications
- Promotes the mission, vision, and values of NYCHHC/Kings County, and abides by service behavior standards.
- Creates and prepares reports using statistics and graphics with detail quality, appropriateness, and consistency with best practices.
- Communicates performance improvement activities to executive leaders through presentations or reports.
- Research literature or external benchmarks/data in support of performance improvement.
- Oversees development, design, and or maintenance of databases for Performance Improvement.
- Analyzes findings and recommends performance improvement initiatives and/or corrective actions.
- Identifies performance and process improvement opportunities to improve CMS, DOH, HEDIS, URAC, and other agencies accreditation and regulatory standards.
- Supervises the staff assigned to departmental activities or units and provides professional and administrative directions.
- Maintains confidentiality of all information as well as the release of any information.
- Performs other activities as assigned by the Chief Quality Officer or Deputy Chief Quality Officer.
Minimum Qualifications1. Master's degree from an accredited college or university in Quality Management, Hospital Administration, Health Care Administration, or in a related health care specialization; and five (5)years of experience in the development of organizational strategies and implementation of staffing plans, regulatory survey preparation and compliance, and in performance improvement and/or continuous quality improvement (CQI) initiatives in a hospital or health care setting, three (3) years of which must have been in a responsible administrative, managerial or supervisory capacity; or,
2.Bachelor's degree in the disciplines as described in "1" above; and seven (7) years of related experience, as listed in "1" above, five (5) years of which must have been in a responsible administrative, managerial or supervisory capacity; or,
3.Bachelor's degree in the disciplines as described in "1" above; and two (2) years of clinical, patient careor equivalent experience in a hospital or health care setting, plus three (3) years of related experience,as listed in "1" above, one (1) year of which must have been in a responsible administrative, managerialor supervisory capacity; or
4.A satisfactory equivalent of education, training, and/or experience. Certified Professional in Healthcare Quality (CPHQ) may be substituted for one (1) year of experience, however, all incumbents must have at least a bachelor's degree and one (1) year of responsible administrative, managerial or supervisory experience, as described in "1" above.
