Director of Patient Care Services (DPCS) - Home Health

This is an exciting opportunity for an experienced home health leader to join us during a time of growth and help shape the continued development of our clinical operations. The DPCS will provide leadership and oversight for patient care delivery, clinical staff, quality, and the overall patient experience.

We're looking for an experienced home health Registered Nurse who understands the unique demands of providing care in the home and has demonstrated success leading clinical teams, managing patient care delivery, and promoting quality, compliance, and an exceptional patient experience.

What You'll Do

As the Director of Patient Care Services, you will provide leadership and oversight for the agency's clinical operations, including:
  • Direct and oversee the delivery of all direct and indirect patient care services.
  • Lead, support, and supervise clinical staff to promote accountability, engagement, and high-quality care.
  • Oversee patient assignments, staffing, scheduling, and clinical coverage to support timely and efficient care delivery.
  • Ensure new referrals are appropriately reviewed and assessment visits are completed timely.
  • Provide clinical leadership in the assessment of patients and development of individualized plans of care.
  • Routinely review clinical documentation and medical records for quality, completeness, accuracy, and timeliness.
  • Monitor patient care and clinical performance to identify opportunities for improvement.
  • Investigate adverse events, unusual occurrences, accidents, and concerns related to patient care or medical devices.
  • Ensure appropriate follow-up, documentation, and corrective action when clinical or operational concerns are identified.
  • Conduct clinical record reviews and provide coaching and education to strengthen documentation and care delivery.
  • Manage clinical staff performance, including ongoing feedback, development, and performance improvement when needed.
  • Collaborate with agency leadership and interdisciplinary team members to support patient outcomes, regulatory compliance, and operational goals.
  • Serve as a clinical resource to staff and help resolve complex patient care issues.
  • Promote a culture centered on quality, collaboration, accountability, and exceptional patient care.

What We're Looking For
  • Current, unrestricted Registered Nurse (RN) license in the state of California.
  • Minimum of three years of experience in a home health agency.
  • Previous experience in a clinical supervisory, management, or administrative capacity within home health.
  • Strong understanding of home health clinical operations, patient care coordination, and documentation requirements.
  • Demonstrated ability to lead, coach, and develop clinical staff.
  • Excellent interpersonal, verbal, and written communication skills.
  • Strong organizational, problem-solving, and decision-making abilities.
  • Ability to establish productive working relationships with patients, families, clinicians, physicians, and agency leadership.
  • Ability to make home visits and provide skilled nursing assessments and care when necessary.
  • Valid driver's license, reliable transportation, and automobile insurance.
