Position Summary

Heritage Crossings is seeking an experienced and forward-thinking Director of Nursing (DON) to lead clinical operations across our skilled nursing facility while providing professional RN oversight for our assisted living community.

This leadership position works in close partnership with the Licensed Nursing Home Administrator (LNHA) to support operational excellence, clinical outcomes, regulatory compliance, and financial stewardship. While daily assisted living operations are managed by an experienced Licensed Practical Nurse (LPN), the DON serves as the RN resource and consultant, ensuring quality standards, compliance requirements, and resident safety remain at the forefront of care delivery.

Key Responsibilities

Clinical & Operational Leadership

  • Lead and oversee all nursing services and clinical operations within our 5-Star skilled nursing facility.
  • Partner closely with the Nursing Home Administrator to align resident care, operational priorities, and financial objectives.
  • Promote excellence in resident outcomes while supporting a culture centered on quality and accountability.
  • Maintain the organization's strong regulatory standing and commitment to exceptional survey performance.

Assisted Living Support & Oversight

  • Provide RN consultation, supervision, and clinical guidance for the assisted living community.
  • Collaborate with the Assisted Living LPN Manager regarding resident care concerns, assessments, incident reviews, and care planning.
  • Monitor compliance with applicable regulations and clinical standards across both service lines.

Quality & Compliance

  • Help lead Quality Assurance and Performance Improvement (QAPI) initiatives campus-wide.
  • Support survey readiness efforts and partner with leadership during state and regulatory inspections.
  • Ensure policies, procedures, and clinical practices consistently meet or exceed regulatory expectations.

Culture & Team Development

  • Mentor nursing leaders, licensed staff, and care teams to promote professional growth and engagement.
  • Foster teamwork and collaboration across departments.
  • Contribute to an environment that reflects our Great Place to Work culture and values.

Qualifications

Required

  • Active Registered Nurse (RN) license in Nebraska.
  • Minimum of 3 to 5 years of leadership experience as a Director of Nursing or Assistant Director of Nursing in a skilled nursing setting.
  • Demonstrated success managing regulatory compliance and state survey processes.
  • Strong leadership, communication, and team-building abilities.

Preferred

  • Bachelor of Science in Nursing (BSN).
  • Master's degree in Nursing, Healthcare Administration, or a related field.
  • Experience collaborating closely with Nursing Home Administrators and interdisciplinary leadership teams.
  • Proven ability to coach and support LPN managers, charge nurses, and clinical leaders.
