Position OverviewThis position is responsible for the overall leadership, management, and quality of nursing services within the community. The role ensures high quality, resident-centered care is delivered while maintaining compliance with regulations.
Essential Job Responsibilities: Resident Care- Plan, organize, and execute all nursing services and clinical operations.
- Ensure compliance with federal, state, and local regulations as well as company policies and procedures.
- Develop, implement, and monitor resident care programs and clinical protocols.
- Perform pre-admission assessments of potential new residents.
- Meet with every new resident and complete basic health assessment within one week of moving into the community and update service plan within thirty (30) days.
- Work with team to oversee and evaluate resident care plans, documentation, and outcomes, reviewing for compliance with state and federal regulations.
- For at-risk residents, coordinate the assistance plan, activate resources through home health, and communicate with the physician and responsible party regarding issues that keep the resident at risk.
- Provide clinical oversight of the memory care program (if applicable), driving team accountability to maintain company memory care programming standards.
- Address resident and family concerns promptly and effectively.
- Serve as primary liaison with regulatory agencies during surveys and inspections.
- Based on physician orders, guide, evaluate, perform, and supervise resident care plans including but not limited to, assessments, resident/family teaching, development and updating of residents' plans of assistance.
- Review incident reports for completeness and follow up documentation in medical record.
- Lead quality assurance and performance improvement initiatives.
- Establish a medication plan that works for each resident so that residents can remain independent in taking medications, and complete 90-day medication reviews.
- Communicate changes in resident's overall condition to the responsible party, physician, Executive Director, and other team members.
- Assume on-call responsibilities and address medical emergencies as they arise.
- Monitor safety and security measures within the department.

Staffing- Recruit, hire, train, and supervise nursing department team members.
- Provide leadership, mentorship, and performance management to the nursing team.
- Delegate appropriate nursing tasks while retaining accountability.
- Observe and document non-licensed personnel skills as indicated.
- Provide team members with in-services on health conditions related to aging, behavioral support strategies, and state, federal, and/or company standards.
- Address team performance or standard of conduct deficiencies swiftly and in compliance with company standards.
- Responsible for the yearly evaluation of medication policies and procedures to include competency/training of nursing staff providing medications.
- Help ensure the overall staffing pattern of the department, assuming on call responsibilities as needed.
Operation of Community/Maintaining Census- Create and maintain an environment for residents, families and guests that is customer-friendly and that values and respects the uniqueness of each individual.
- Market the community by developing professional contacts in the local area.
- Encourage lifestyle activities to promote socialization amongst residents, assisting with the occasional activity
- Attend regularly scheduled staff meetings and in-services.
- Report on-the-job injuries to the supervisor before the end of the work shift on the day the injury occurs.
- Maintain confidentiality of verbal and written information pertaining to residents, community operations and personnel.
Education, Experience, and Other Requirements- Active RN license in state where working
- Minimum of 3-5 years nurse management experience in senior living, long-term care, or assisted living
- CPR certification
Knowledge, Skills, and Abilities Required- Proficient computer skills
- Strong knowledge of state clinical practices, state and federal regulations pertaining to assisted living and/or memory care, and quality standards
- Ability to manage multiple priorities in a fast-paced environment
- Excellent assessment, problem solving, and communication skills
- Ability to organize and prioritize with good attention to detail
- Knowledge of HIPAA requirements
- Proficient in electronic health records systems
- Ability to travel occasionally for meetings
