Director of Nursing

The Director of Nursing (DON) leads all nursing operations, ensuring exceptional care and service for clients. You will manage and develop nursing staff, coordinate care plans, and maintain compliance with all regulatory standards. This role combines leadership, clinical expertise, and a commitment to delivering compassionate, high-quality care.
Essential Functions

Client Care
  • Manage health care of all clients and communicate with families, staff, and physicians.
  • Facilitate continuity of care for clients receiving home health services.
  • Assess clients on a routine and as-needed basis; document and report changes.
  • Share 24/7 on-call responsibility for nursing coverage.
  • Notify staff and families of client condition changes and treatments.
  • Coordinate with other departments to ensure consistent, proactive care.
  • Develop and update individualized care plans.
  • Provide client and family education on care and procedures.
  • Stay current on nursing practices through continuing education.
  • Respond to emergencies in alignment with nursing protocols.

Leadership & Supervision
  • Direct and supervise nursing staff, providing coaching and performance feedback.
  • Train, educate, and develop nursing staff; delegate responsibilities appropriately.
  • Review and maintain client charts and medication records.
  • Monitor medication administration for accuracy.
  • Address and resolve client, family, and staff concerns.
  • Promote company values and represent the organization positively.

Administration & Compliance
  • Monitor and enforce infection control procedures.
  • Review incident reports and medication errors; provide follow-up training.
  • Maintain TB records in compliance with state regulations.
  • Conduct pre-admission screenings and complete admission documentation.
  • Maintain Home Care Provider Client Roster and nursing attendance records.
  • Complete required documentation, assessments, and care plans.
  • Perform other duties as assigned.
Required Qualifications
  • Graduate of an accredited school of nursing.
  • Active Minnesota RN license in good standing.
  • Proficient in English (written and verbal) and computer skills.
  • Strong assessment, communication, and leadership skills.
  • Knowledge of federal, state, and local nursing regulations.
  • Ability to work under pressure and handle emergencies.
  • Current First Aid, CPR, and AED certification.
  • Reliable transportation and valid driver's license.
  • Ability to lift/carry 30+ lbs. and meet physical requirements.
  • Availability for on-call duties and emergency response.
Preferred Qualifications
  • Experience with older adults.
  • Minimum three (3) years of nursing leadership experience.
