Liberty Health Care Center is seeking a strong, experienced, and compassionate Director of Nursing (DON), RN, to lead the clinical operations of our skilled nursing facility.

What You'll Lead
  • Clinical Operations & Nursing Leadership
  • Oversee all nursing operations within the Skilled Nursing Facility
  • Lead and support RN, LVN, and CNA teams
  • Promote a culture focused on quality care, teamwork, professionalism, and accountability
  • Mentor and develop nursing leadership staff, including ADONs, Unit Managers, and Charge Nurses
  • Ensure proper staffing management and clinical oversight
  • Quality Measures & Regulatory Excellence
  • Lead Quality Assurance & Performance Improvement (QAPI) initiatives
  • Drive improvement in CMS Quality Measures and Five-Star performance
  • Monitor and improve:
    • falls
    • wounds
    • infections
    • rehospitalizations
    • weight loss
    • medication compliance
  • Maintain ongoing survey readiness and regulatory compliance
  • Infection Prevention & Clinical Compliance
  • Oversee infection prevention and antibiotic stewardship initiatives
  • Lead audits, Plans of Correction (POCs), and compliance efforts
  • Ensure adherence to all CMS, state, and federal regulations
  • Staff Development & Retention
  • Recruit, develop, and retain strong nursing staff
  • Support employee engagement and clinical education initiatives
  • Foster a positive, solutions-focused culture within the nursing department

What We're Looking For
  • Current, active Texas RN license in good standing (required)
  • Prior Director of Nursing or Assistant Director of Nursing experience in a Skilled Nursing Facility REQUIRED
  • Minimum 5+ years of progressive nursing leadership experience in LTC/SNF
  • Strong working knowledge of:
  • CMS regulations
  • survey process
  • QAPI
  • infection prevention
  • staffing management
  • clinical reimbursement/documentation
  • Proven leadership ability with strong communication and organizational skills
  • Experience improving Quality Measures and clinical outcomes preferred
  • PointClickCare experience preferred
