Liberty Health Care Center is seeking a strong, experienced, and compassionate Director of Nursing (DON), RN, to lead the clinical operations of our skilled nursing facility.

What You'll Lead- Clinical Operations & Nursing Leadership
- Oversee all nursing operations within the Skilled Nursing Facility
- Lead and support RN, LVN, and CNA teams
- Promote a culture focused on quality care, teamwork, professionalism, and accountability
- Mentor and develop nursing leadership staff, including ADONs, Unit Managers, and Charge Nurses
- Ensure proper staffing management and clinical oversight
- Quality Measures & Regulatory Excellence
- Lead Quality Assurance & Performance Improvement (QAPI) initiatives
- Drive improvement in CMS Quality Measures and Five-Star performance
- Monitor and improve:
- falls
- wounds
- infections
- rehospitalizations
- weight loss
- medication compliance
- Maintain ongoing survey readiness and regulatory compliance
- Infection Prevention & Clinical Compliance
- Oversee infection prevention and antibiotic stewardship initiatives
- Lead audits, Plans of Correction (POCs), and compliance efforts
- Ensure adherence to all CMS, state, and federal regulations
- Staff Development & Retention
- Recruit, develop, and retain strong nursing staff
- Support employee engagement and clinical education initiatives
- Foster a positive, solutions-focused culture within the nursing department
What We're Looking For- Current, active Texas RN license in good standing (required)
- Prior Director of Nursing or Assistant Director of Nursing experience in a Skilled Nursing Facility REQUIRED
- Minimum 5+ years of progressive nursing leadership experience in LTC/SNF
- Strong working knowledge of:
- CMS regulations
- survey process
- QAPI
- infection prevention
- staffing management
- clinical reimbursement/documentation
- Proven leadership ability with strong communication and organizational skills
- Experience improving Quality Measures and clinical outcomes preferred
- PointClickCare experience preferred
