General Responsibilities
Leads the recruitment, training, supervision, motivation, and evaluation of team members to ensure the delivery of high-quality, person-centered care in alignment with the organization's mission and values.
Effectively plans, organizes, and delegates work assignments, ensuring appropriate scheduling and coverage.
Provides case management services for residents, including pre-occupancy assessments and developing and overseeing individualized care plans.
Coordinates all aspects of resident move-ins, including medication setup and orientation to the community.
Demonstrates respect for residents' heritage, values, and life experiences, and a strong understanding of the aging process.
Investigates resident and staff incidents, identifies safety risks, and implements timely corrective actions to ensure a safe, supportive environment for all.
Regularly monitors and maintains accurate records of residents' physical and mental health conditions, promptly reporting any significant changes to the appropriate physician.
Reviews resident care plans to ensure quality of care, documents service delivery, and leads staff in implementing necessary adjustments.
Conducts regular team meetings to promote communication and continuous improvement.
Oversees the safe and accurate administration of residents' medications.
Documents residents' responses to medications and ensures follow-through on all physician orders, including verbal changes.
Accurately documents any incidents involving residents.
Coordinates wellness clinic operations and ensures timely health screenings and follow-ups.
Actively supports and participates in the resident-centered activity program as duties permit.
Collaborates effectively with all team members to deliver coordinated, compassionate resident care.
Conducts onboarding and orientation for new team members.
Develops and delivers in-service and ongoing training programs to maintain a high standard of care.
Encourages and facilitates constructive problem-solving and communication among team members.
Performs other duties as assigned in support of resident care and organizational operations.

Must be a licensed nurse
Must possess strong interpersonal skills and the capability of relating to various people and personalities. Considerable initiative, judgment, and leadership skills are also required.
Must demonstrate the ability to hire, direct, and manage personnel.

Physical Working Requirements
Exposed to body fluids, infection, odors, and behavior of residents.
Exposed to chemical/cleaning solutions.
Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately to meet the position requirements.
Must be able to bend, stretch, reach, lift or move up to 25 pounds at a time, turn and walk. Occasionally lift/move up to 50 pounds.
