Full Job Description
Director of Community Health Improvement to provide strategic leadership for community health, population health, prevention, and outreach initiatives across our service area. This is a unique opportunity to help address the root causes of poor health, build meaningful partnerships, and create a lasting impact for the communities we serve. The Director oversees Community Health Improvement, Prevention Services, and Blueprint for Health teams. As a key organizational leader, you will work closely with healthcare providers, community organizations, state agencies, and regional partners to improve health outcomes and advance initiatives that support healthier, more resilient communities.

About This Role

As the Director of Community Health Improvement, you will lead NVRH's efforts to improve population health by integrating social determinants of health into healthcare delivery and community initiatives. You will oversee Vermont Blueprint for Health programs, Accountable Care Organization (ACO) population health efforts, Community Health Needs Assessments (CHNAs), community benefit programs, grants, and strategic partnerships designed to enhance the well-being of people throughout the Northeast Kingdom. This role combines strategic leadership, community engagement, program development, grant oversight, and collaborative partnership building to ensure NVRH continues to meet its mission of improving the health of the communities we serve.

Key Responsibilities
  • Lead community health improvement initiatives, including Vermont Blueprint for Health, ACO population health programs, Accountable Health Community efforts, and collaborative community partnerships focused on improving health outcomes.
  • Direct Community Health Needs Assessments (CHNAs), implementation strategies, annual action plans, community benefit activities, and regulatory reporting requirements.
  • Oversee community health grants, program budgets, staff, performance metrics, and funded initiatives while ensuring compliance, accountability, and achievement of program objectives.
  • Build and maintain relationships with healthcare organizations, government agencies, community partners, businesses, and stakeholders to address social determinants of health and advance community well-being.
  • Represent NVRH through public speaking engagements, community meetings, health education programs, and regional and state-level health initiatives.
  • Collaborate with hospital leadership to identify emerging community health needs, evaluate outcomes, develop new programs, and establish strategic priorities.

Required Qualifications

  • Experience overseeing grant-funded programs and community-based health initiatives.
