Full Job Description
What You'll Do

As the Director of Clinical Services, Home Health, you will lead the delivery of clinical services while ensuring compliance, quality, and operational success.

Responsibilities include:
  • Providing overall direction and oversight of home health clinical services
  • Ensuring compliance with all applicable state and federal home health regulations
  • Coordinating and overseeing all direct and indirect patient care services
  • Managing the development, implementation, and review of individualized patient care plans
  • Collaborating with physicians and interdisciplinary team members to coordinate patient care
  • Supporting and mentoring clinical managers and staff to promote professional growth and performance improvement
  • Assisting with budget preparation, financial planning, and resource management
  • Monitoring operational indicators, census trends, staffing needs, and productivity metrics
  • Overseeing recruitment, onboarding, performance evaluations, and employee development
  • Ensuring accurate maintenance of clinical records, reports, and documentation
  • Managing inventory, supplies, and equipment required for patient care services
  • Leading orientation, training, in-service programs, and continuing education initiatives
  • Driving quality assurance and performance improvement activities across the organization
  • Assisting in the development and implementation of organizational goals, policies, and procedures
  • Promoting home health services and fostering relationships within the healthcare community
  • Serving as acting Home Health Executive Director in the Executive Director's absence
  • Participating in administrative on-call rotations
  • Providing clinical support to cluster partners and collaborating across teams
  • Performing other duties as assigned to support organizational goals and patient care excellence


What We're Looking For:
  • Current licensure in the State of Oklahoma as a Registered Nurse (RN), Physician, Physical Therapist, Occupational Therapist, Speech-Language Pathologist, or Social Worker
  • Three years of management experience in home health, hospice, or a related healthcare setting preferred
  • Demonstrated leadership experience supervising clinical and administrative personnel
  • Strong knowledge of home health regulations, compliance requirements, and healthcare operations
  • Excellent interpersonal, leadership, coaching, and team-building skills
  • Ability to analyze operational and financial data to support informed decision-making
  • Strong verbal, written, and organizational communication skills
  • Ability to build relationships with referral sources, patients, families, and community partners
  • Valid driver's license, reliable transportation, and proof of insurance in accordance with agency requirements
  • Commitment to delivering high-quality, patient-centered care and supporting clinical excellence
