Qualifications

• Knowledge of PDPM and regulatory requirements.
• Previous experience in case management is advantageous.

Responsibilities

• Facilitate daily care coordination meetings to align resources with patient needs.
• Lead Engage/72 HR Admission meetings as part of the care management system.
• Manage the integration of the restorative nursing program with activities.
• Communicate timely updates to patients and families regarding care changes.
• Collaborate with the clinical team on care plans development and execution.
• Coordinate discharge planning and post-discharge communications.
• Monitor quality measures with the care coordination team.
• Ensure care documentation meets Medicare and Managed Care guidelines.
