POSITION SUMMARY:
The Care Link Clinic - Director is responsible for the operational aspects of the clinical function. The director will plan, program and use the resources of the practice organization to assure its goals are met. The director provides Vice President of Transitional Care Management with information and data necessary to formulate policy and implements general and clinical policy and procedures. The director will work under the direct supervision of the Vice President of Transitional Care Management.
POSITION EDUCATION/ QUALIFICATIONS:
• Texas Registered Nurse (R.N.) license required
• BSN Degree required
• BLS Certification
• ACLS Certification required within 90 days of hire
• PALS Certification required within 90 days of hire, as applicable to department
• Minimum of three (3) years of hospital experience in a progressively responsible position requiring contact with various departments.
• Familiarity with clinic equipment, systems and procedures.
• Capability to manage clinical and non-clinical staff.
• Good written and verbal communication skills required.
• Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians
• Demonstrated leadership experience
• Ability to read, write and speak English required
• Ability to speak Spanish preferred
JOB KNOWLEDGE/EXPERIENCE:
• Knowledge of State and Federal (CMS) regulations.
• High degree of clinical competency/experience in general.

Responsibilities:
POSITION RESPONSIBILITIES:
PRACTICE MANAGEMENT
• Assists with Hire New Personnel - Interviews, tests, verifies information on employment applications and checks references. Gives new employees appropriate orientation following the personnel checklist.
• Assigns Duties - Organizes personnel and assigns duties pertaining to medical records, patient care, reception and housekeeping.
• Training - Ensures all employees have an in-depth knowledge of the jobs assigned to them. Seeks input from employees as to how jobs can be improved, combined or eliminated. Helps to create job descriptions where none exist.
• Payroll -Ensures that accurate records are kept of all hours worked. Reviews, makes edits and approves all time sheets. Ensures that all information and any changes in information are sent to the Payroll Department at DHR in a timely manner.
• Schedules - Establishes vacation schedules and approves all sick and emergency leaves for employees prior to them adding it to the Kronos system for approval, keeping in mind the needs of the practice. Rearranges staffing to cover for the absent employee.
• Policy Implementation - Responsible for implementing new policies, processes or procedures to the staff.
• Performance Evaluations - Schedules and conducts annual and 90 day performance and salary reviews for employees.
• Employee Relations - Fields all questions, concerns and conflicts. Handles all employee suggestions and complaints; counsels employees as needed. Responsible for the positive motivation of employees and ensures that all employees are treated equally.
• Personnel Records - Maintains a "shadow file" for all employee records. Performs audit of files to ensure compliance.
• Personnel Laws -Understands the various state and federal laws that govern employment and personnel actions. Works closely with hospital HR dept. on any employee issues for advice and resolution, after going through training.
• Staff Meetings - Schedules and conducts regular staff meetings with all employees.
• Employee Termination - Recommends termination of employees to HR after discussion with MDs and Administrators with documentation as needed.
CLINIC MANAGEMENT
• Clinical Review - Responsible for chart reviews for readmission assessment and appropriateness of care
• Patient Care - Responsible for conducting RN clinical assessment, patient education, and follow-up phone call as needed.
• Maintenance - Responsible for supervising the maintenance of the clinic and to assure its neatness.
• Purchasing - Responsible for the purchasing of clinic supplies, equipment, printed materials and the coordination of purchase orders through the Materials Management Dept at DHR.
• Equipment - Responsible for overseeing equipment maintenance and ensuring proper working order.
• Computers - Responsible for the maintenance and upkeep of the personal computers for the clinic. Trouble-shooting when within knowledge range; calling for support when necessary. Will train and answer employee questions as needed.
• Meetings - Schedules and attends regular staff meetings; meetings with
Vice President of Transitional Care Management; and committee meetings as assigned.
• Compliance - The Director must understand Compliance Regulations / HIPAA Regulations and is responsible for training the staff with the help of the Compliance Dept at DHR. The Director is to ensure that the clinic is in compliance in order to meet TJC requirements, and to work their clinics compliance plan.

B. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
