Job Summary
The Director of Care Management & Social Services is responsible for the operations of CHOC Children's Case Management and Social Services Departments. Works with various systems and departments to ensure improvements in coordination of care and care delivered across the continuum. Administers all patient care services including oversight and development of processes related to complex patient and family psychosocial issues provided by the Case Management and Social Services Department. Serves as a resource to CHOC and the community regarding services provided by the departments.

Pay Range
Minimum $154,918 Midpoint $205,234 Maximum $255,590

Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.

Experience
• Required: Minimum five (5) years of clinical experience. At least three (3) years of management experience.
• Preferred: Experience in Case Management, Managed Care, or Community Clinic setting.

Education
• Required: Graduation from an accredited school of Registered Nursing. Bachelor's degree in Healthcare and Master's degree in health care or business.

Training
• Required: Experience with process improvement, analysis, and change implementation of business processes.

Specialized Skills
• Required: Possesses and demonstrates the ability of advanced communication and interpersonal skills with all levels of internal and external customers including but not limited to medical staff, patients and families, clerical personnel, support and technical staff, outside agencies, and members of the community. Demonstrates sufficient analytical ability to interpret cost data. Ability to develop, prepare and or interpret graphics, statistics, and financial data. Mental stability to sufficiently cope with complex problems relative to patients, staff, physicians, visitors, or external agencies. Independent judgment required to solve complex clinical problems, with the ability to develop strategies to cope with changing health care environment and balance the patient care institutional needs in relation to available resources. Competent in Microsoft Office products including Word, Excel, and MS project. Ability to set goals and judge results in accordance with the highest standards; advance care through development of new ideas and technology; promote teamwork to achieve CHOC's mission; and understand and exceed customer expectations.
• Preferred: Bilingual. Demonstrates desire and ability to work with children and families in a multi-cultural environment.

Licensure
• Required: Basic Life Support (BLS) - required within 30 days of employment or transfer; Registered Nurse - CA (RN).
• Preferred: Certified Pediatric Nurse (CPN).

Work Environments - Functional Demand:
