Qualifications

• Degree in nursing, therapy, or related clinical field; must hold a current RN license.
• MDS experience essential for the role.
• Knowledge of healthcare reimbursement and utilization management.
• Proven experience in strategic leadership and team management.
• Ability to develop and execute strategic plans effectively.
• Strong communication, both verbal and written, is vital.
• Familiarity with PDPM requirements and regulatory standards is mandatory.

Responsibilities

• Facilitate daily meetings to align care resources with patient needs.
• Lead and participate in Engage/72 HR Admission meetings.
• Manage and integrate the restorative nursing program with activities.
• Communicate timely with patients and families about care changes and discharge.
• Collaborate with the clinical team on care plan development and execution.
• Coordinate discharge planning and post-discharge communication.
• Monitor Quality Measures and support the care coordination team.
• Ensure accurate and timely completion of care documentation.
