Your Role

The Commercial Care Management team performs integrated case management (CM) and disease management (DM) activities demonstrating clinical judgment and independent analysis by collaborating with members and those involved with their care including a multidisciplinary team and the member's treating physicians. The Registered Dietitian will report to the Commercial Care Management Manager. In this role you will be responsible for supporting the overall Commercial CM team as a subject matter expert while exemplifying BSC leadership behaviors.

Your Work

In this role, you will:
  • The Registered Dietitian will serve as a subject matter expert in nutrition, overseeing the medical nutrition therapy of the most complex and/or high-risk patients.
  • Apply detailed knowledge of Commercial HMO, PPO, ASO and Blue Shield of California's (BSC) established medical/departmental policies, clinical practice guidelines, community resources, contracting and community care standards to each case.
  • Assess nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans and provide nutritional counseling in coordination with members' physician.
  • Consult with members' physicians to determine nutritional needs and diet restrictions of patient or client.
  • Advise patients and their families on nutritional principles, dietary plans and diet modifications, and food selection and preparation.
  • Counsel individuals on basic rules of good nutrition, healthy eating habits, and nutrition monitoring to improve their quality of life.
  • Keep up-to-date clinically and apply this new knowledge to your job.
  • Observe, receive, and otherwise obtaining information from all relevant sources.
  • Provide nutritional counseling to ensure patient's growth and success
  • Determine the need to add, discontinue, or change dosing and timing of nutrition related medications to minimize drug/nutrient interactions in conjunction with member's physician.
  • Recommend member participation and assist members to enroll in pilots approved by the BSCA related to nutrition


Qualifications

Your Knowledge and Experience
  • Bachelor's degree in Didactic Program in Dietetics (DPD) required
  • Master's degree in nutrition preferred
  • Registered Dietitian required
  • Minimum of 5 years experience working as a Dietitian in a community clinic, or other home and community- based organization
  • A minimum of (5) years experience coaching patients with chronic disease conditions, such as Diabetes, CAD, Asthma, COPD, CHF, Cancer and other Rare Disease Conditions
  • Demonstrated ability to independently assess, evaluate, and interpret clinical information and care planning
  • In depth understanding of community resources, treatment options, home health, funding sources and special programs
  • Extensive knowledge of evidenced based clinical practice guidelines particularly for chronic conditions.
  • Strong analytical and problem-solving skills
  • Strong written and oral communication skills


Hybrid Virtual Work

This role allows employees to work virtually full-time, however employees will be expected to come to the office based on business need.
