Summary

Collaborates with members of the healthcare team to provide Medical Nutrition Therapy for Home Infusion patients of all ages. Facilitates discharge planning and coordinating nutrition care for nutrition support patients including triage for inpatient/outpatients.

Position Reports To: Nutrition Therapy and Diabetes Director

Essential Functions

  1. Assess and provide nutrition prescriptions, recommendations and education for pediatric and adult Home Infusion patients including oral nutrition supplements, enteral formulas and TPN.
  2. Provide pump education and methods of nutrition administration for inpatients and outpatients.
  3. Works closely with families, care givers and health professionals to provide optimal nutrition for high risk patients.
  4. May include home visits or Telehealth.
  5. Utilizes community resources and programs to assist patients in their plan of care.
  6. Coordinates and facilitates the Nutrition Support Care Plan throughout the patient stay, including discharge preparation, planning, and transition to outpatient Home Infusion services.
  7. Collaborates with Case Management, Medical, Nursing, Ancillary and Home Infusion staff to eliminate barriers to efficient delivery of care.
  8. Ensures that all elements critical to the Nutrition plan of care has been communicated to the patient/family or receiving facility, in collaboration with the Interdisciplinary team.
  9. Coordinate the assessment, nutrition care plan, education and supplies for Nutrition Support patients outside of acute care requiring Home Infusion services.
  10. Complete chart notes per organizational policy.
  11. Ability to function independently and as a member of a cross-functional team. Able to organize and set work priorities in a multi-task environment.
  12. Participates in LEAN performance improvement and research activities.
  13. Required to maintain active Basic Life Support (CPR) certification.
  14. Other duties as assigned.
