- No weekends
- Fixed schedule/no on-call
Key Responsibilities:- Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
- Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
- Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
- Develops and maintains a plan of care that describes interventions to attain expected outcomes;
- Implements and records interventions to attain expected outcomes in EMR;
- Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
- Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
- Maintains a positive and respectful attitude to all patients and colleagues;
- Occasional travel to other nearby locations; and
- Medication administration and documentation.
Requirements:- Valid RN license
- BS/AA/diploma in nursing
- 6 months of RN experience strongly preferred
- 3 months of hemodialysis experience strongly preferred
- Valid drivers license and transportation
- Ability to travel covering multiple locations
- BLS certification (American Red Cross or American Heart Association Required); and
- Ability to lift up to 35 pounds is required
- This is a safety sensitive position

Compensation:Salary Range: $40.00 - $60.00 per hour. *Based on experience.*Compensation offers are based on education, experience, skills, and geographic region.
