We are seeking a full-time 40-hour Diabetes Program Coordinator to support our Internal Medicine suite Monday through Friday from 8:00-5:00pm. The schedule consists of 4 days in Haverhill and 1 day at our Andover location.
Summary
Responsible for overseeing the successful planning and completion of educational programs for Patient care.

Essential Functions
-Maintain accreditation for the program.
-Conduct individual and group patient education sessions; maintain the schedule of patient appointments.
-Coordinate reporting for audits and implement recommendations as a result of the audits.
-Maintain records pertaining to program activities.
-Purchase or approve the purchase of equipment and supplies needed by the staff and program.
-Schedule program work, oversee daily operations, coordinate the activities of the program, and set priorities for managing the program.
-Manage marketing and outreach to the community.
-Prepare or assist in the preparation of grant requests and funding continuation from outside sponsors.
Education
Bachelor's Degree Related Field of Study required

Licenses and Credentials
Registered Nurse [RN - State License] - Generic - HR Only preferred
Basic Life Support [BLS Certification] - Various Issuers preferred
Certified Diabetes Care and Education Specialist [CDE/CDCES] - Certification Board for Diabetes Care and Education (CBDCE) preferred
Experience
Health Education &/or Literacy Experience 3-5 years required
