Full Job Description
Preferred Candidate: Registered Nurse (RN) or Registered Dietitian (RD)/Registered Dietitian Nutritionist (RDN)

Summary:

Provides comprehensive assessment, patient/family education, and facilitates behavior change for patients in the field of diabetes mellitus in 1:1 counseling & small group sessions.

Interacts and communicates effectively with members of the patient's diabetes management team (i.e. patients PCP, endocrinologist, podiatrist) to ensure optimal diabetes care.

Serves as resource and diabetes clinical expert for Virtua staff, Virtua VMG, Virtua POEs.

Position Responsibilities:
• Counsel patients/families on diabetes self-management (either in 1:1 setting or in small group setting) incorporating ADA National Standards for Diabetes Self-Management Education and AADE adapted Behavioral Change Goals. Makes follow-up telephone calls and/or emails to patients as necessary.
• Documentation on patient records, EMR, Diabetes Database and written communication to patient's medical provider(s) in accurate, timely, & professional manner.
• Provides diabetes wellness education via Virtua diabetes support groups, and community events.
• Participates in diabetes program & education curriculum development & revision.
• Serves as clinical diabetes resource by responding to requests for information & clinical updates from Virtua staff, physicians, schools, etc.
• Participates in new staff orientation and student intern observational experiences.
• Attends & participates in bimonthly staff meetings and other meetings as needed.

Position Qualifications Required / Experience Required:

2+ years' experience in diabetes patient education, knowledge and comfort with facilitation of group patient education, strong communication and interpersonal skills .

Required Education:

Bachelor's degree required, Masters preferred.

Training/Certifications/Licensure:

CDCES (Certified Diabetes Care and Educational Specialist) by the National Certification Board for Diabetes Educators (NCBDE). Obtain certification within 1 year of becoming eligible.

May be necessary to be a Certified pump trainer, ie experience with Insulet , Medtronic and Tandem insulin pumps preferred.

Annual Salary: $75,177 - $116,950 The actual salary/rate will vary based on applicant's experience as well as internal equity and alignment with market data.
