Position at Palomar Health

Description

Facilitates daily operations of the unit and provides structure, safety, and emotional support to clients in acute psychiatric crisis. Ensures that the unit is maintained and that basic services are provided. Under direct supervision of an RN, maintains the basic services of the unit such as, meals, laundry and housekeeping provided on a daily basis. Ensure that all repairs are made to meet safety and health standards, assist clients with daily living activities, coping strategies, personal hygiene, laundry, etc., and provides personal supervision and monitoring of clients to ensure client's health and safety is being maintained. Monitor, reinforce, and assist clients to follow through with their crisis treatment plan and document client's progress as directed. Responsible for providing direct patient care for adult patients in a Crisis Stabilization Unit (CSU). Maintains appropriate verbal, personal space, and relational boundaries with clients. Provides educational group support, recreational activities, and contributes to and supports the therapeutic milieu. Participates in the gathering of information, monitors patient progress, and contributes to, implements and documents the individualized patient plan of care, provides case management, and assists in discharge planning, and follow-up phone calls as needed to follow patients through the system. Works with Navigators to provide a warm hand-off for high-risk patients. Participates as a team member of the clinical interdisciplinary team to ensure appropriate care. Supports and provides resources to all internal/external customers and displays on-going commitment to quality. Responsible for safeguarding the welfare of employees, patients, and visitors. Speaks and reads English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, PowerPoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job. Performs other duties as assigned. Follows Palomar Health rules, policies, procedures, applicable laws and standards. Carries out the mission, vision, and quality commitment of Palomar Health.
