Work Arrangement: Remote with frequent travel to Avardis Health centers
Travel Requirement: Approximately 50-75%, including overnight travel and occasional extended stays

About the Role

We are seeking an experienced Corporate Director of Social Services to provide strategic leadership, operational oversight, education, and field support for Social Services departments across our portfolio of skilled nursing and rehabilitation centers. This leader will partner with regional and center teams to strengthen regulatory compliance, resident and family experience, care transitions, behavioral health support, and quality outcomes.

The ideal candidate is a collaborative, results-oriented long-term care leader who can develop people, standardize systems, analyze performance, and support facilities through surveys, transitions, and improvement initiatives. This role is both a coach and an operational partner, helping teams deliver consistent, resident-centered care the Avardis way.
What You'll Do
  • Develop and standardize Social Services processes, policies, tools, and documentation across Avardis facilities.
  • Guide admissions support, psychosocial assessments, resident adjustment, discharge planning, care transitions, grievance management, and family communication.
  • Provide leadership on CMS requirements, Social Services-related F-Tags, resident rights, abuse prevention, PASRR, care planning, behavioral health, and trauma-informed care.
  • Support survey preparation, complaint investigations, Plans of Correction, regulatory recovery efforts, and IDR/IIDR activities when appropriate.
  • Coach and mentor facility Social Services Directors through onboarding, competency validation, education, performance improvement, and career development.
  • Champion resident satisfaction, family engagement, resident and family councils, behavioral health programming, and quality-of-life initiatives.
  • Monitor performance indicators including readmissions, successful community discharges, grievances, satisfaction, care-plan compliance, behavioral health outcomes, and survey trends.
  • Lead Social Services-related QAPI initiatives and use data to identify and sustain improvement opportunities.
  • Partner with Operations, Nursing, Therapy, Admissions, Business Office, Human Resources, and medical providers to create a seamless resident journey.
  • Provide on-site support during leadership vacancies, surveys, transitions, high-risk situations, and operational improvement efforts; serve in an interim facility role when needed.
Minimum Qualifications
  • Master's degree in Social Work.
  • Active Social Worker license where required by state regulations.
  • At least five years of Social Services leadership experience in skilled nursing or long-term care.
  • Extensive knowledge of CMS regulations and requirements affecting Social Services.
  • Strong expertise in discharge planning, care transitions, resident rights, behavioral health, and psychosocial services.
  • Excellent leadership, coaching, communication, relationship-building, and conflict-resolution skills.
  • Ability to analyze data, lead change, manage projects, and work independently across multiple priorities.
  • Ability and willingness to travel approximately 50-75%, including overnight travel and occasional extended stays.
Preferred Qualifications
  • Experience supporting multi-site or multi-state operations.
  • Experience with behavioral health programming.
  • Survey and regulatory response experience.
  • QAPI leadership experience.
