Qualifications

  • Thorough knowledge of HomeTown LTC Consultant Pharmacist Manual and relevant regulations.
  • Proficiency with Rxpertise consulting software.
  • Geriatric Certification preferred with additional 2-3 years of experience.
  • Membership in ASCP (American Society of Consultant Pharmacy) and commitment to ongoing education.

Responsibilities

  • Build and maintain positive relationships with clients and facility staff.
  • Attend QA, Behavioral Management, and Pain Management committee meetings as needed.
  • Conduct thorough chart reviews at a target rate of 60-70 charts per day after initial training period.
  • Prepare and present clinical reports to facilities promptly post-consult.
  • Be available for consults during facility surveys, in person or by phone.
  • Collaborate with nursing staff to address clinical concerns during facility visits.
  • Contribute to HTP LTC newsletter by writing clinical articles and training future consultants.

Full Job Description
Consultant Pharmacist

The Consultant Pharmacist position is key in building and maintaining positive relationships with each of our clients. Our main goal is to be a caring, friendly and personal health care resource for them. We go the extra mile for our clients representing our HTP culture. Our consultant pharmacists represent HomeTown Pharmacy and lead by example in nurturing/maintaining the HomeTown culture.

The job expectations are as follows:

Thorough knowledge of the HomeTown LTC Consultant Pharmacist Manual.

Thorough knowledge of Rxpertise consulting software.

Thorough knowledge of all Guidance to Surveyors F-tag regulations concerning pharmacy services.

Be available for facility Quality Assurance, Behavioral Management, Pain Management or other necessary committee meetings.

Meet with the Director of Nursing at each facility visit to review general clinical information, and specific findings or concerns.

Perform chart reviews at a rate of 60-70 charts per 8 hours after 2-3 months in a facility.

Print out reports with proper breakouts (nursing, medical director etc.) and present to facility shortly after consult is completed.

Be available via phone, or in person, if requested during facility surveys.

Act as a HomeTown Pharmacy LTC service representative while in each facility, inquiring on the level of satisfaction with the entire scope of pharmacy services.

Be a member of ASCP (American Society of Consultant Pharmacy) and stay current on all clinical developments. Strive for Geriatric Certification with 2-3 years.

Contribute clinical articles to the HTP LTC newsletter.

Work with pharmacy school clinical students, train new consultants.

Build relationships with physicians and other prescribers to ensure that each patient receives optimal drug therapy. Work with prescribers to ensure support of HTP Therapeutic Interchange Program.

Work in the pharmacy according to Dispensing Pharmacist Job Description.

Must know how to conduct Comprehensive Medication Reviews and other MTM activities effectively.

Be part of the HomeTown LTC on-call rotation.
