General Summary:

The Manager of Community Outreach provides strategic leadership for the hospital's volunteer program and community engagement portfolio, advancing the organization's mission through coordinated outreach, partnerships, volunteer services, community health worker programs and mobile health initiatives. This role strengthens the hospital's presence and impact across the communities it serves while ensuring alignment with Community Benefit priorities and regulatory requirements. Working closely with executive leadership, internal partners and community stakeholders, the manager translates identified organization and community needs into measurable, high-impact strategies that improve access, enhance patient experience and support long-term community trust.

Responsibilities:

1. Provides strategic leadership for enterprise-wide community outreach initiatives aligned with the organization's mission, Strategic Plan, Community Benefit objectives and Community Health Needs Assessment (CHNA) priorities.
2. Leads the tri-annual CHNA process, including community and partner engagement, survey design, data analysis and development of the implementation strategy.
3. Conducts ongoing assessment of community health trends, social determinants of health and existing community resources to identify gaps, risks and opportunities for health system engagement.
4. Oversees planning and execution of community-based outreach, health education and engagement initiatives targeted to priority populations and identified community needs.
5. Provides strategic oversight of the organization's mobile health unit(s), including outreach planning, community deployment, partner coordination and alignment with identified community priorities.
6. Identifies, cultivates and manages strategic partnerships with community organizations, agencies and coalitions that address current and emerging health and social needs within the organization's primary service area.
7. Provides stewardship and oversight of Bayhealth's financial contributions, sponsorships and grants supporting community outreach and health improvement initiatives, ensuring alignment with strategic priorities and measurable impact.
8. Ensures coordination and alignment of community engagement efforts across the health system and affiliated entities to maximize effectiveness and minimize duplication.
9. Serves as a primary liaison between internal leaders and community partners, facilitating collaboration and mobilizing resources to address shared community priorities.
10. Provides strategic direction and oversight for community health worker programs designed to improve access, navigation and connection to care and community resources.
11. Ensures community health worker activities are aligned with identified community needs, internal referral pathways and organizational goals related to access, equity and community engagement. Partners with clinical, operational and community stakeholders to integrate community health workers into appropriate workflows and outreach initiatives.
12. Monitors all community-based program effectiveness, outcomes and resource utilization, and adjusts strategy to ensure sustainability and impact.
13. Provides vision, leadership, advocacy and oversight for Volunteer Services, ensuring volunteer programs support patient experience, hospitality and community engagement goals.
14. Develops and strengthens meaningful volunteer-patient experience relationships that enhance engagement, connection and perception of care, with a focus on positively influencing patient experience and engagement metrics.
15. Ensures volunteer programs are appropriately recruited, trained, deployed, and recognized to support a consistent, high-quality experience for patients, families and visitors.
16. Leads enterprise-wide Community Benefit reporting, including oversight of CBISA tracking, education of leaders and staff, monitoring of compliance and production of required reports.
17. Oversees development of the annual Community Benefit Report in collaboration with Finance, the Bayhealth Foundation and Marketing & Communications.
18. Collaborates with Marketing & Communications to support strategic communications and public service campaigns related to community health priorities and outreach initiatives.
19. Ensures compliance with all applicable regulatory, accreditation and reporting requirements related to community benefit, outreach, mobile health services and volunteer management.
20. Leads, mentors and evaluates assigned staff across community outreach, community health worker programs and volunteer services, fostering a culture of service excellence, accountability and professional development.
21. Develops and manages the annual operating budget for the community outreach department and provide oversight and approval of the Volunteer Services budget.
22. Establishes performance metrics and reporting to evaluate program effectiveness, community impact and return on investment.
23. Performs other duties as assigned within the scope and responsibility of the role.

Required Education, Credential(s) and Experience:

  • Education: Bachelor Degree Public Health Or Bachelor's Degree in Health Administration, Healthcare Management, Social Work, Community Health, Nonprofit Management, Communications, or a related field.
  • Credential(s): None Required;
  • Experience:
    Required: Seven years of progressive experience in community outreach, community engagement, population or community health programs, nonprofit or healthcare-based partnerships, or related fields. At least 3 years of leadership experience managing teams, programs, or cross-functional initiatives in a healthcare, nonprofit, or community-based organization. Demonstrated experience leading or supporting Community Health Needs Assessments (CHNA), community benefit initiatives, or community-based program evaluation. Experience managing community partnerships, sponsorships, grants, budgets or financial stewardship related to outreach or community benefit activities.
    Preferred: 10 or more years of progressive experience in community outreach, community engagement, population or community health programs, nonprofit or healthcare-based partnerships, or related fields. Experience overseeing or collaborating with volunteer services and/or community-based workforce programs (e.g., community health workers, navigators, outreach staff).
