As a Home Health Occupational Therapist, you'll help patients overcome the barriers that keep them from living independently. Every home presents a different challenge, every patient has a unique goal, and every visit is an opportunity to restore confidence-not just function.

What You'll Do

  • Perform occupational therapy evaluations in patients' homes.
  • Develop individualized treatment plans that improve independence with activities of daily living.
  • Assess home safety and recommend adaptive equipment and environmental modifications.
  • Educate patients and caregivers on techniques that improve safety, function, and quality of life.
  • Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
  • Complete timely, accurate documentation while monitoring patient progress.

What We're Looking For
