Full Job Description
Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking a CM Project Administrator - Specialty Program to join our team.

Primary Responsibilities:

  • The Care Management Project Administrator collaborates in the design, implementation and evaluation of care and disease management projects and specialty programs.
  • Serves as primary point of contact and resource for specified projects, collaborates with stakeholders, assists with troubleshooting issues, prepares deliverables for submission, and oversees project-related contract details.
  • Prepares, submits, and presents reports on project performance at internal and external meetings, as needed.


Tasks Performed:

  • Supports the development and management of projects and initiatives by creating project plans using existing tools to ensure that project requirements, deadlines and schedules are submitted timely.
  • Collaborates with stakeholders to:
    • 1) Identify and develop necessary workflows and processes
    • 2) Develop and implement documentation and associated reporting systems
    • 3) Manage project oversight


  • Develops, reviews, and revises resources/toolkits/playbooks for designated projects.
  • Tracks and reports on project milestones and outcomes.
  • Supports leadership and teams to conduct analyses and refine processes and programs.
  • Researches published scientific literature, evidence-based guidelines, and other sources of credible information pertinent to clinical program management, makes recommendations to leadership for program modifications, and supports implementation.
  • Presents for adoption clinical content/evidence-based guidelines.
  • Collaborates with others to create, implement, and evaluate quality improvement activities.
  • Serves as a resource for provider practices on designated projects to troubleshoot issues and address system related barriers.
  • Leverages expertise in care management to support navigation of HUSKY Health benefits, care coordination requirements, and preparation for healthcare encounters.
  • May engage with eligible members identified for CHNCT care management services, including conducting an assessment and developing a care plan, when relevant to the specific project.
  • Coordinates with other CHNCT departments and ASOs to ensure access to care/services.
  • Participates in collaborative meetings that align with the goals of the project/program.
  • Advises and participates in the development of audit criteria and conducts audits, as needed.
  • Supports fulfillment of URAC requirements and participates in re-accreditation survey process.
  • Performs other duties as assigned.


Essential Functions:

  • Development and execution of project plans for implementation of special programs.
  • Participation in workgroups and meetings as they relate to projects and programs.
  • Coordination with stakeholders to ensure that deliverables occur on schedule. Travel to designated provider/community sites on a scheduled frequency and as needed.
  • Preparation and presentation of project-related reports.
  • Maintain knowledge of covered CT Medicaid/HUSKY Health programs.


Desired Education: Four to six years post-secondary education

Desired Degree: Bachelor's degree; Master's degree preferred

Desired Major: Nursing

Desired Job Experience: 5+ years Care Management program experience required.

Other Qualifications: Data Analytics and System Expertise. Excellent verbal, written and organizational skills required. Independent forward thinker. CCM or other URAC approved certification preferred. Must hold a valid and unrestricted motor vehicle operator's license and have reliable transportation. CT Registered Nurse (RN) license must be current and unrestricted.

Certifications Required: Active Registered Nurse License, State Motor Vehicle License
