Program DescriptionThe Transition Age Youth Full-Service Partnership (TAY FSP) Program provides intensive community-based High Fidelity Wraparound services to 30 young adults (ages 16-25) identified as having the highest mental health-related needs in San Mateo County. The program relies on a diverse staff and innovative program model to effectively meet the needs of this underserved and often marginalized population. Specific supports and services provided by this team include case management and care coordination, mental health treatment (assessment, therapy, medication management, and psychiatry), family support, crisis prevention and intervention, skill building (independent living, vocational, community safety, and emotional/behavioral), socialization and recreational activities, academic support and coordination, employment exploration, basic needs, social support, and housing support.
Position DescriptionWorking collaboratively with a multi-disciplinary leadership team, the Clinical Program Managerprovides direct supervision of Case Managers and TAY Specialists. This includes oversight of the case management and adjunctive care components of the program; and coordination between TAY treatment teams, Drop-in-Centers, Edgewood departments, and external partners. It creates, plans, and oversees content-specific training, facilitates team meetings, creates and implements program procedures and processes, and interfaces with county providers and system of care managers.
The Clinical Program Managercoordinates referrals from intake to exit of the TAY program. This includes review of new referrals, coordination of referral documentation from external partners, initial contact with new clients, coordinating and facilitating initial meetings with clients, external referents, and internal providers, treatment team assignments, and support with initial intake paperwork. This position also supports clients' exits from the program including appropriate referrals, closing paperwork support, and facilitating connection to new providers. This position shares responsibility of client charts, records, and data collection with the Regional Clinical Supervisor.
Core Competencies- Recruit, train and supervise Case Managers and TAY Specialists, both individually and in groups. Providing ongoing coaching and support to staff to maintain a positive, safe, and empowering experience for clients.
- Teach, coach, and mentor direct reports in the areas of: case management, behavior coaching, independent living skills training, employment support, educational access, meeting facilitation, strengths and needs assessment, advocacy, crisis prevention and intervention, professional communication, outreach and engagement, conflict resolution, Medi-Cal documentation, and care coordination.
- Review and approve documentation (progress notes, assessment, case plans) of all supervisees.
- Synthesize referral information, caseload size and workload, cultural preferences/considerations, staff competencies, and young adult preferences to make appropriate assignments.
- Engage system and community partners in dialogue to deepen their understanding of the unique needs, barriers, and perspectives of young adults.
- Identify gaps, create procedures, and devise systems for quality improvement.
- Design systems to evaluate and audit case files.
- Ability to plan and facilitate groups and meetings that encourage engagement and perspectives of all.
- Manages intake/referral process for the TAY FSP program, including the collection of documents for the referral process, following up with referents and completing documentation for opening a reporting unit (RU) in AVATAR.
- Responsible for outreach to and engagement with San Mateo County BHRS (Behavioral Health and Recovery Services), Probation, and HSA (Human Services Agency) for referral generation and follow up.
- Acts as liaison between Edgewood, BHRS, Probation and HSA.
- Collaborates with FSP leadership and program teams to enhance internal systems related to referrals, care coordination, and graduation practices and policies.
- Acts as a consultant for TAY providers and managers on housing supports of planned and emergency shelter options, hospital discharge planning, and accessing housing-related community resources.
- Coordinate with San Benito Apartments property manager
- Acts as liaison between client teams and MHA regarding placement in the San Benito apartments (deposit, rent, lease, etc.)
- Creates/maintains housing agreements, subsidized rent agreements, housing protocols, and housing needs assessments.
- Trains TAY FSP staff on vital housing options, functions, and protocols.
- Demonstrate clinical expertise and leadership when providing consultation/supervision to treatment teams.
- Manage and monitor all required paperwork of staff within established timelines, meeting all guidelines; ensures that charts and documentation are organized and clinically prepared for audit by agency, San Mateo County, and Medi-Cal.
Essential Functions- Sets strategic goals, creates a plan, and leads the process to achieve these goals.
- Ensures direct reports complete their documentation and required paperwork within established timelines, including meeting monthly efficiency as required for the position.
- Completes role-specific documentation and paperwork thoroughly and in a timely manner, meeting expected timeframe and productivity requirements.
- Regards TAY clients as persons with dignity and competence; encourage and mentor staff and county partners in viewing this population as full collaborators in service planning, delivery, and evaluation.
- Effectively initiate, communicate, and collaborate with TAY FSP program leadership on program service delivery and components, training, evaluation and outcomes, new projects, and team cohesion.
- Demonstrates a range of crisis prevention intervention approaches, providing leadership, consultation and training to staff and teams.
- Anticipate the implication and consequences of situations/ choices and take appropriate action to prepare for possible contingencies; tackle problems by using logical, systematic, and strength-based approaches.
- Attends and participates in all mandatory meetings, including staff meetings, client team meetings, and treatment planning meetings.
- Understands and values cultural and racial differences, their alternative perspectives on mental illness, help-seeking and alternative healing practices, as well as lifestyles, goals, family, and community life.
- Maintains confidential health records of young adults in compliance with the Edgewood, EPSDT, and San Mateo County Behavioral Health standards and regulations.
- Attends and actively participates in 1:1 supervision; enlists support, requests, and integrates feedback.
- Maintains positive work relationships in a respectful and collaborative manner.
- Collaborate and communicate with internal and external stakeholders, in a manner that promotes agency mission, vision, and values; agency, region, and area goals; consumer and family satisfaction; and highest quality care.
- Participate in week-long rotation of Manager on Call (MOC), providing direct support to crisis response workers who are covering the Full-Service Partnership After-Hours Crisis Line.
- Other duties as assigned.
- Provide direct services as needed.
Applied Knowledge, Skills, and Abilities- Bilingual (English/Spanish) preferred.
- Education/Experience: Master's degree in clinical psychology/social work and at least two years of experience in direct services role or management of direct service; at least twof these years in a case management or care coordination role with high-risk individuals or families.
- Clinical License Preferred: Licensed Marriage and Family Therapist, Licensed Clinical Social Worker, or Licensed Professional Clinical Counselor.
- Experience with at-risk, disenfranchised, and/or marginalized populations.
- Strong leadership qualities: strengths-based, time management, organization, ability to coordinate and motivate others, teamwork, and facilitation of team environment.
- Must have a dependable car, valid California driver's license, and proof of insurance.
