Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

• Follows-up with patients in a timely manner and monitors for drug therapy response, potential adverse drug reactions, allergies, compliance, laboratory results, and other monitoring measures as appropriate; treatment plan adjustments are made accordingly based on assessed measures.
• Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, pharmacokinetic impact, and therapeutic outcomes as required; information is communicated with team members and appropriate alternatives to current treatment plans are provided.
• Promotes and monitors compliance for established drug therapy policies.
• Coordinates with and refers to appropriate diagnosing providers when disease progression or events occur that are outside the Endocrine CPP provider SOP.
• Documents appropriate subjective and objective information, as well as clinical interventions in the electronic medical record and in a timely, professional manner.
• Reports and documents drug errors, adverse drug reactions, and allergies according to policy.
• Conducts drug utilization reviews, quality assurance activities, and utilization reviews as required.
• Instructs patients and family members effectively in the appropriate use of medications and medical devices, adjusts communication and tracking methods based on age and developmental considerations, accurately assesses and documents patient comprehension, and refers patients to available educational programs.
• Plans, develops, administers, and implements educational programs to enhance clinical skills and knowledge of professional pharmacy staff, medical, providers, pharmacy, residents, and pharmacy interns.
• Maintains an effective liaison with staff members of all professional disciplines and services as consultant regarding pharmaceutical problems, and pharmacy operating policies and procedures.
• Coordinate and perform selected medication utilization evaluations as requested by the Chief of Pharmacy.

GENERAL DESCRIPTION OF ASSIGNED DUTIES

The Endocrine CPS is responsible for comprehensive medication management which may include but is not limited to CGM/insulin pump management, bariatric surgery, U-500 insulin, osteoporosis, thyroid/PTH, and familial lipids. This CPS may also serve as a therapeutic consultant on matters of high degree of complexity. Services provided may be conducted via multiple modalities, including virtual, telephone and in-person care. The Endocrine CPP provider functions at the highest level of clinical practice and works independently under their scope of practice (SOP) to directly care for patients. A collegial relationship with mutual consultation and referral exists with the collaborating provider(s) and the CPS. Consultation with an endocrinologist or appropriate referring provider is required for advanced patient care management beyond the CPS's scope of practice (e.g., diagnosis). The primary duties are to provide comprehensive medication management to Veterans.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

• Citizenship. Citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• English Language Proficiency. Pharmacists must be proficient in spoken and written English.
• Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy). Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
• Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.(1)

Grade Determination: GS-13 Clinical Pharmacy Specialist

• Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
• Pharmacists assigned to this position must demonstrate the following KSAs:
  
  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • Expert understanding of regulatory and quality standards for their program area.
  • Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred Experience: PGY-2 in Ambulatory care, 3+ years experience in Ambulatory Care (PACT/CPP), or board certified ambulatory care pharmacist (BCACP/Board certified pharmacotherapy specialist (BCPS)
