Qualifications

Responsibilities

  • Assist in pharmacy educational programs and mentor residents.
  • Present drug evaluations and provide input to the Pharmacy & Therapeutics Committee.

New Opportunity! Clinical Pharmacy Specialist-Emergency Medicine - $15k sign-on bonus and $4k relocation bonus!

Hours/Shift: Full-Time/Rotating

Key Responsibilities:
  • Provides clinical pharmacy services for our patients and serves as a resource, educator, mentor, and leader to physicians and other caregivers.
  • Coordinates and provides clinical pharmacy activities, including but not limited to pharmacokinetics, parenteral nutrition and general pharmaceutical care in all patient care areas as needed.
  • Identify, resolve, and prevent potential/actual medication-related problems including:
    • Initiating, adjusting, monitoring, and recommending changes in drug therapy based on information obtained through collaborative practice; interdisciplinary rounding; patient interviews; objective monitoring parameters; review of drug profiles and patient medical records.
    • Coordinate and interpret laboratory monitoring of drug therapy, including but not limited to serum drug levels, electrolytes, and other approved drug-related laboratory testing.
    • Perform clinical pharmacy activities to decrease overall patient healthcare costs by effective pharmacotherapy, resulting in decreased length of stay, hospital readmission rates and overall better patient clinical outcomes.
  • Assist in clinical pharmacy or educational programs; serves as a mentor and preceptor in the residency training program.
  • Presents drug monographs and reviews, medication use evaluations, policy, and provides input to Pharmacy & Therapeutics Committee to support efficacious and cost-effective drug therapy.
  • Maintains interface with medical staff to provide information for new pharmaceutical supplies, formulary, drug usage, and compatibility and new regulations regarding clinical drug use.
  • Performs other duties as assigned to support overall effectiveness of department and organization.


Qualifications:
  • Education: Graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree (Pharm.D.).
  • Certification: PGY2 Resident in Emergency Medicine/Critical Care or Board of Pharmacy Specialties Certification (BCEMP or BCCCP)
    License: Licensed in the State of Arizona or within 90 days of hire the license is granted.


Work Requirements: Ability to sit 50% of time; stand/walk 50%; ability to grasp, lift, reach, stoop, repetitive movement; carry or lift objects weighing up to 10 pounds; ability to feel, see, hear, smell, speak, and distinguish colors.
