Job Description:Performs all pharmacist functions, actively participating in established standard work parameters and providing coverage of all pharmacy areas. Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults. Leads and facilitates pharmacy practice programs, direct patient care services, and medication systems. Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems. Verifies and enters medication orders, proactively communicating with medical staff to resolve problems. Provide direct patient care, including pharmacotherapy assessment, therapeutic monitoring, pharmacokinetic dosing, and transitions-of-care. Precept, mentor, and educate pharmacy students, residents, and staff, and provide continuing education to interdisciplinary team members. Contribute to scholarly activity and professional advancement through publications, presentations, and committee involvement. Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors. Accurately manages medication inventories, and prepares and dispenses patient-specific medication doses. Leads and facilitates pharmacy practice programs, and directs patient care services and medication systems. Other duties as assigned.
Knowledge, Skills, and Abilities: Education: - Bachelor's [Required]
- Doctorate [Required]
Field of Study: Work Experience: - 3+ pgy1 residency plus contemporary hospital or specialty practice experience consistent with practice needs of institution [Required]
- 5+ clinical pharmacy experience in contemporary hospital or specialty practice consistent with practice needs of institution [Required]
- Pharmacy leadership experience within a hospital setting or professional organization [Preferred]
- Practice experience in a large hospital (greater than 500 bed institution) and integrated healthcare system (three or more hospitals in system) [Preferred]
Additional Information: Licenses and Certifications: - Licensed Pharmacist (RPH) [Required]
- Basic Life Support - CPR Cert (BLS) [Preferred]
- Advanced Cardiac Life Support Cert (ACLS) [Preferred]
