Full Job Description
Position Summary
Under the general direction of the Manager of Inpatient Pharmacy Programs, provides pharmaceutical care according to standards established by the Department of Clinical Pharmacy to all patients. Documents the care provided to patients. Participates in policy-development committees as assigned (both intra-departmental and interdisciplinary committees). Serves as preceptor to pharmacy students and residents as assigned. Participates in the development of institution-wide drug use policy. Responsible for ensuring safe and efficient drug distribution and control system. Participates in supervision of technical staff. Maintains open communications and is available to component organizations and practitioner sites for consultation.

Core Job Functions
Medications are dispensed according to established procedure and State/Federal regulations. Performs clinical services to optimize drug therapy. Clinical assessment decisions are made to include appropriate cost-effective therapy to ensure excellent patient outcomes.
Provides consultation/intervention on non-formulary drug requests. Patients are counseled regarding dosage therapy, side effects and interactions related to discharge medications. Provides in-service education to hospital personnel regarding new medications as needed
Inpatient medication profile is monitored for drug interactions, food/drug interactions, duplicate therapy, and other potential medication problems in drug therapy. Assures that central pharmacy, nursing stations and ancillary areas use appropriate methods for medication storage.
Solutions are prepared, labeled and available according to policy guidelines and physician orders. Assures that aseptic technique is followed during preparation.
Provides supervision and instruction of PhamrD students on clinical rotations as assigned. Precepts PharmD Residents as assigned providing supervision and mentorship. Participates in the planning and evaluations of PharmD students and Residents as needed.
