Job Description

This position will be working primarily weekdays with one Saturday coverage a month.

We are seeking a Staff Pharmacist to support our Long-Term Care pharmacy, which serves as the primary contracted pharmacy for the Oregon PACE program and Providence Child Center. In this role, you will play a critical part in ensuring safe, effective, and high-quality medication therapy for geriatric and pediatric populations in a facility-based care environment.

Key Responsibilities:
  • Interpret, verify, and process physician medication orders with accuracy and timeliness
  • Monitor drug therapy to ensure safety, appropriateness, and optimal clinical outcomes
  • Provide pharmaceutical care for long-term care patients, including geriatric and pediatric populations
  • Collaborate closely with physicians, nurses, clinical pharmacists, and care teams within the Oregon PACE program and Providence Child Center
  • Ensure compliance with all professional standards, regulatory requirements, and pharmacy best practices
  • Support and manage cycle fill processes, Medication Administration Records (MARs), and facility-based medication systems
  • Provide clinical recommendations and drug information to healthcare providers
  • Perform medication reconciliation during routine cycle fills


Required Qualifications:
  • Education to meet certification, license or registration requirement.
  • Upon hire: Oregon Pharmacist License
  • 1 year Pharmacist associated experience or completion of a PGY-1 residency preferred


Preferred Qualifications:
  • Previous experience in Long-Term Care (LTC), PACE programs, institutional pharmacy or retail pharmacy setting
  • Board certification or advanced clinical training
  • Experience working in interdisciplinary healthcare teams
  • Familiarity with electronic health records and pharmacy systems used in LTC settings
  • Comfortable working in a high-volume prescription environment
