Job DescriptionThis position will be working primarily weekdays with one Saturday coverage a month.
We are seeking a Staff Pharmacist to support our Long-Term Care pharmacy, which serves as the primary contracted pharmacy for the Oregon PACE program and Providence Child Center. In this role, you will play a critical part in ensuring safe, effective, and high-quality medication therapy for geriatric and pediatric populations in a facility-based care environment.
Key Responsibilities:
- Interpret, verify, and process physician medication orders with accuracy and timeliness
- Monitor drug therapy to ensure safety, appropriateness, and optimal clinical outcomes
- Provide pharmaceutical care for long-term care patients, including geriatric and pediatric populations
- Collaborate closely with physicians, nurses, clinical pharmacists, and care teams within the Oregon PACE program and Providence Child Center
- Ensure compliance with all professional standards, regulatory requirements, and pharmacy best practices
- Support and manage cycle fill processes, Medication Administration Records (MARs), and facility-based medication systems
- Provide clinical recommendations and drug information to healthcare providers
- Perform medication reconciliation during routine cycle fills
Required Qualifications: - Education to meet certification, license or registration requirement.
- Upon hire: Oregon Pharmacist License
- 1 year Pharmacist associated experience or completion of a PGY-1 residency preferred
Preferred Qualifications:- Previous experience in Long-Term Care (LTC), PACE programs, institutional pharmacy or retail pharmacy setting
- Board certification or advanced clinical training
- Experience working in interdisciplinary healthcare teams
- Familiarity with electronic health records and pharmacy systems used in LTC settings
- Comfortable working in a high-volume prescription environment

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
