Responsibilities

  • Lead clinical areas and therapeutic programs for assigned patient service lines
  • Provide direct patient care through decentralized and service-based programs
  • Monitor and evaluate patients' medication regimens for safety and efficacy
  • Communicate findings and recommendations to clinicians about medication therapy
  • Mentor and precept Doctor of Pharmacy students and pharmacy residents
  • Conduct in-service education for medical, pharmacy, and nursing staff
  • Engage in research collaborations and hospital quality assurance programs

37.5 Hours Per Week

Qualifications

Graduation from an ACPE-accredited school of pharmacy with a Doctor of Pharmacy (PharmD) degree Completion of ASHP-accredited PGY-1 Pharmacy Residency (required) Completion of ASHP-accredited PGY-2 Residency in specialty area (preferred); clinical pharmacy experience (at least 2-4 years) in a hospital setting and in relevant area of practice (required if no PGY-2 residency) Board Certification by the Board of Pharmacy Specialties in primary area of practice (e.g., Pharmacotherapy (BCPS), Emergency Medicine (BCEMP), Pediatric Pharmacy (BCPPS)) upon hire or within 12 months of hire (required) Meet ASHP residency preceptor requirements (required)

Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certification upon hire or within 3 months of hire (required); Pediatric Advanced Life Support (PALS) if applicable based on area of practice

New York State Pharmacist Licensure and Registration (required) Excellent written and verbal communication skills. Demonstrates initiative and leadership skills.

Strong analytical and problem-solving skills.

Proficient in Microsoft Office, including preparing PowerPoint presentations.

Speaks, reads and writes English to the extent required by the position; knowledge of a second language preferred.

USD $79.89 - USD $86.93 /Hr.
