Compensation: $131,000 - $141,000 annually

Summary:

The Clinical Pharmacist ensures the safe, effective,cost-conscious use of medications by providing expertise in the therapeuticbenefits, drug interactions, adverse reactions, and disease-specificevidence-based treatment protocols common to primary care and specialtymedicine. The Clinical Pharmacist will interact with patients, clinicians,nurses, and other health care providers to provide management, information, andeducation about the use of drugs.

Principal Responsibilities:

1. Will participate and maintain a culture within TheCorvallis Clinic that is consistent with the content outlined in the Serviceand Behavioral Standards document. To this end, employee will be expected toread, have familiarity, and embrace the principles contained within.
2. Work providing direct patient care. Obtain and evaluate patient medicationregimens, allergies, and past/current medical history. Design, recommend andmonitor patient specific therapeutic regimens with evidence-based medicine. Usecollaborative drug therapy management protocols for disease state management.Examples include diabetes, hypertension, hyperlipidemia, and pain management.
3. Participate and support guideline, pathway, and other disease managementefforts. Participate in providing drug use feedback to providers usingavailable data.
4. Collaborates with nurse care managers to provide expert advice onprescribing practices for selected patients with complex and unstable diseaseto improve their health and prevent unnecessary hospital and emergency carecosts.
5. Conduct target drug programs or other interventional efforts designed tooptimize the use of pharmaceuticals.
6. Professional Practice: Remains current in pharmaceutical regulations andapplicable laws and advises The Corvallis Clinic to ensure compliance.
7. Perform other duties as assigned.

Education, Licensure and Experience:

1. Bachelor's degree in Pharmacy RPh or a PharmD degree froman accredited college or university required.
2. Oregon Pharmacy licensure required.
3. Pharmacy practice residency or a minimum of three (3) years practiceexperience in direct patient care required.
4. Experience in an ambulatory care clinic or inpatient setting preferred.

Knowledge and Skills:

1. Ability to communicate well in an interdisciplinary teamsetting.
2. Lead and train where necessary and accurately represent the standards ofpharmaceutical practice.
3. Flexibility and vision to try new collaborative approaches to clinicalproblems and the willingness to persevere through the challenges of creatingpositive change in a medical clinic setting.
4. Ability to read, analyze and interpret professional journals, technicalprocedures, or governmental regulations.
5. Ability to interpret a variety of instructions furnished in written, oral,diagram or schedule form. Demonstrates ability to set goals, plan, organize,and take tasks to completion.
6. Exercises independent judgement within the limits of authority.
7. Thorough knowledge of Electronic Medical Record, Spreadsheet and WordProcessing software.
