Qualifications

• Licensed Pharmacist (RPH) required.
• Bachelor's degree required; PharmD preferred.
• 1+ years of hospital experience preferred.
• Demonstrated competency in pharmacy informatics and medication management.
• Strong organizational and interpersonal skills.

Responsibilities

• Educate patients and healthcare teams on medication safety and use.
• Oversee medication therapy management and participate in pharmacy stewardship programs.
• Lead interdisciplinary rounds and ensure patient care continuity from admission to discharge.
• Mentor pharmacy technicians, students, and residents in their professional development.
• Identify and address medication safety issues and adverse reactions.
• Engage in quality improvement and compliance initiatives, monitoring performance metrics.
• Perform all aspects of pharmaceutical care including compounding and pharmacokinetic dosing.
